The Crisis & Home Treatment team within Derbyshire CAMHS is a new and exciting element of our Urgent Care Provision.

This post is for the role of Non-Medical Prescriber within the Crisis & Home Treatment Team. The post holder will be expected to provide holistic assessment of children and young people experiencing a mental health crisis, manage risks associated and prescribe accordingly.

The team will be a multi-disciplinary one inclusive of Band 4, 5 & 6 Practitioners, Consultant Psychiatrist & Trainee ACP. The post holder will work alongside another Senior Practitioner within team, leadership roles across the urgent care provision and CAMHS leadership.

The post holder will support the management of medication across the CAMHS Crisis Team, in conjunction with consultant psychiatrists from all areas of CAMHS.

The CAMHS Service is able to offer a robust induction to prescribing within CAMHS and support any training needs the Non-Medical Prescriber may have in order to fulfil the role.

Main duties of the job

The post holder will be able to provide senior knowledge and experience within the team and take a senior position in the MDT to formulate & implement care plans and consider prescribing options.

Provide clinical leadership and prescribing in complex situations that require diplomacy, negotiation skills, and sound clinical judgement and to include assessment of risk, risk mitigation and a holistic assessment of needs. As well as this, you will also be required to provide direct support to more junior members of the team and act as a senior member of the service area.

You will undertake delegated responsibilities, ensuring systems are in place to effectively manage resources and you must be able to work in conjunction with senior leaders within the CAMHS Leadership Structure to challenge existing practice and initiate improvement in services to meet the needs of Children and Young People (CYP) and achieve corporate and national objectives. Maintaining a CYP-IAPT compliant service.

You will provide highly specialist clinical input to the team, delivering practice within best evidence and to the highest possible standard as well as working alongside NMP's across the service to ensure evidence based service delivery is being coordinated as per local & Trust policy & procedure.

• To offer robust case management and mental health support to those children and young people presenting in a mental health crisis.

• You will identify the safest treatment options including pharmacological treatment and as indicated prescribe these having reached concordance with the patient and following liaison with the patients GP/Consultant.
• You will maintain your competence (knowledge and skills) to be able to offer the most appropriate medication including being aware of its mode of action, being able to identify potential side-effects of treatment, being able to monitor its effectiveness and carry out any other requirements regarding the medication of choice i.e. physical tests.
• To interpret and analyse clinical and non-clinical information to form an accurate assessment and prognosis across a wide range of conditions, to recommend the best course of intervention, and to support others to do the same. Evaluating patient progress, reassess and alter treatment programs if necessary. To maintain accurate, comprehensive and up to date records/documentation through a paper and/or computerised system in line with agreed standards.
• To communicate assessment and treatment results to appropriate disciplines verbally or in the form of reports and letters, this will include representing the service or patient at multi-disciplinary team meetings where patient care, patient progress and discharge may be discussed.
• You will interpret the results and recognise if they indicate that further investigations are required or that it is safe to commence treatment.
• To contribute to the referral and discharge criteria, and develop a system for managing own caseload, which ensures the service is delivered in an accessible way to service users, prioritising vulnerable groups and utilising administration services as available.
• When working in the community or community settings, work as a lone practitioner where applicable and contributing to the management of clinical risk across the service.
• To communicate effectively with patients and carers to maximise the individuals potential and ensure an understanding of the condition; offering education, advice & support to informal and formal carers to best support patients safely.

• To be able to travel the geographical area of Derby City and Southern Derbyshire to attend meetings, statutory reviews, multi-agency network meetings, client consultations and training.
