An exciting opportunity has arisen for a Senior Community Mental Health Nurse to join an established team at Havant Health Centre.

Your main responsibilities will include care coordination of service users, duty work on rotation, and mental health monitoring and supervision and mentorship of junior staff and students.

The post holder will be based at Havant Health Centre and will need to work flexibly. You will also need to establish good working relationships with the local GPs, Italk service and other community and voluntary organisations. We're investing in our mental health services in South East Hampshire, transforming the way we work to provide outstanding care and building truly rewarding careers.

The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.

Due to the nature of this post in order to carry out home visits and attend patient meetings a car driver is essential with access to own car.

Main duties of the job

*To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness including organic and functional illnesses

*To undertake ongoing assessment, treatment and evaluation for service users known to the service.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
