A rare and exciting opportunity has arisen to join the HIV Community Clinical Nurse Specialist team, within the internationally recognised CWHFT HIV directorate.

Acting as a link between CWHFT HIV services and the community setting, you will holistically manage a caseload of people living with HIV, who may have complex physical and psychosocial needs. You will work within a small and dynamic team based from Chelsea and Westminster Hospital, regularly collaborating with other professionals, utilising telemedicine where appropriate and hot-desking from various sites. You will be responsible for visiting patients in their homes, or in other community-based settings, such as, GP surgeries, and drug and alcohol treatment centres, predominantly across the boroughs of Hounslow and Ealing, with a requirement to also provide cross-cover for other boroughs.

The team aims to improve access to, and engagement with HIV treatment and care to reduce unnecessary attendance and admission to healthcare services and to support discharge planning where appropriate. They also seek to improve overall health, wellbeing and quality of life by empowering patients to take a fuller role in their healthcare and in addressing the wider determinants of their health. Main duties of the job

As part of a multidisciplinary community team and in liaison with the acute services at the Chelsea and Westminster NHS Foundation Trust, to enhance the provision of care to people living with HIV/AIDS currently on or off combination drug therapies and to empower individuals to maximise their independence in living as normal a life as possible.

To work both as an independent practitioner, and in partnership with the team, supporting Stable HIV patients across all areas commissioned by Chelsea and Westminster Healthcare NHS Fdn Trust.

To provide proactive case management of patients with HIV with highly complex needs who struggle with adherence and engaging in care.

To act as an expert and a resource on HIV healthcare issues to Specialist Palliative Care Service, local nursing services, GP's and social service teams.

To promote collaboration between acute and community health and social care providers.

To organise and help facilitate teaching and training on HIV related healthcare issues, with specific focus on supporting clients taking complex combination therapy regimes.

To develop and empower the primary care teams to improve their ability to manage clients on combination therapy.

Job responsibilities

Professional Practice
  • In collaboration with the Senior HIV CNS, manage a caseload of stable HIV patients and with highly complex needs within the community setting who have been identified as requiring specialist expertise in the management of Highly Active Antiretroviral Therapy HAART and coordinating complex care packages to assist in maintaining them within the community setting.
  • Ensure that patients are fully informed and supported by providing clinical and social support through patient education around management of HIV and provide education for those newly diagnosed.
  • Work autonomously, independently assess and diagnose problems/issues, prescribe treatment, review blood results, order and interpret clinical tests and deliver and evaluate plans of care, within own sphere of competence and with supervision from the Senior HIV CNS/HIV Consultant colleagues.
  • Effectively deliver an expert, efficient and high quality service to all patients requiring HIV care.
  • Be a key contact point in the service for patients and professionals

Education
  • To identify, assess and meet the educational needs of the acute, primary and social care teams in relation to the specific needs of this client group.
  • Identify, assess and meet the educational health care needs of patients and carers.
  • To participate in teaching and assessing in clinical practice, including mentorship and preceptorship of pre and post registration healthcare professionals in both settings.
  • To continue own professional development in line with NMC Guidelines on Professional Practice.

Research
  • In conjunction with the Senior HIV CNS, undertake research related to the CNS role and the provision of quality healthcare in the community.
  • To use Trust wide and local clinical indicators to measure the quality of nursing care being delivered to patients.
  • Support the implementation of evidence based practice
  • To actively explore opportunities coming out of audit to undertake research within own speciality.

Management
  • To manage a small delegated budget for equipment and supplies pertinent to the role.
  • To maintain a flexible approach to the role and ensure provision of an effective and efficient service within agreed budgets and resources.
  • To contribute to the continuing development of HIV services to attain high quality patient focused care
  • To encourage innovation and develop new approaches to the delivery of patient focused care.

Person Specification

Education and Qualifications

Essential

  • Registered RN with a degree in nursing
  • Degree in Nursing
  • Substantial experience in HIV
  • Mentorship course or equivalent
  • Qualification in the speciality of HIV or a recognised HIV course
  • Adherence and treatment support training
  • Evidence of recent, relevant professional development
  • Skilled in venepuncture


Desirable

  • Independent Nurse Prescriber
  • Masters course or working towards a masters

Experience
