Community Mental Health Services in Harrow have undergone transformation and are pleased to announce the Harrow Triage and Assessment Team. The Adult Mental Health Community redesign is part of an initiative to improve the quality of care received by people with mental health needs in their local community. The national direction of travel, outlined in the NHS Long Term Plan, is towards the provision of more integrated, personalised and place-based care.
Harrow Triage and Assessment Team, based in Harrow and Wealdstone is looking for a passionate mental health registered mental nurse who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities. CNWL is passionate about developing its own staff, driving them to go on to become more established practitioners and this post is no different. Harrow is a pioneering service in Trauma Informed Approachesand all staff receive ongoing training in this area.

- Have an excellent knowledge of Mental Health conditions, therapies and current mental health legislation.
- Be responsible for the full mental health assessment of care needs of patients presenting with a wide variety of clinical conditions. Following this, the post-holder will be responsible for development, implementation and evaluation of programmes of care without supervision, unless otherwise indicated.
- Demonstrate the ability to support and apply a holistic, person-centred, approach to assessing the health and social care needs of people experiencing severe and enduring mental illness, who have been referred to the Hub.
- Have a flexible, multi-disciplinary approach and have a solid understanding of risk assessment, care planning and the recovery model. You will also need to be able to offer support and advice to less experienced colleagues.

- Current Professional Qualification in mental health nursing Registration must be live with professional body.
- Evidence of post registration training and CPD.
Desirable
- Training in Psychosocial Interventions or Thorn Family Therapy/Work training
- Other relevant training, e.g. substance misuse, Training in Care Act 2014, Phlebotomy Training
Experience
Essential
- Experience of working in multidisciplinary mental health team with clients with acute/severe and enduring mental health problems
- Extensive post-registration experience
Desirable
- Work with people experiencing mental health crisis. Audit and research
- Teaching Working with people who use substances
- Working with people who use substances
- Previous experience in Clozapine Clinic
- Work with clients with acute/severe and enduring mental health problems who are homeless/rootless
- Supervision of students and junior staff.

- Excellent verbal and written communication skills, including report writing and presentations / IT skills.
