The main duties of the posts are to assess and review care for individuals with complex health care needs within the community and care homes.

Main duties of the job

An exciting opportunity has a risen for a band 7 post within the CHC & Care Home Governance Team. The successful applicant will have experience across the areas of CHC, safeguarding and care home governance. The position will be aligned to a locality Senior Nurse, and work collaboratively and flexibly with other localities where there are increased work demands, as part of the wider developments to manage the workflows more equitably across these areas of work.

This post holder would be responsible for working jointly with health, social care and independent sector colleagues, and have a key role with monitoring the contracts with care homes and quality of patient safety and the safeguarding agenda providing professional advice, support, guidance to professionals and families concerning all aspects of CHC.

Vital to this appointment are excellent communication and interpersonal skills, working collaboratively with team colleagues, managing priorities of a case load, having excellent analytical skills to interpret information, preparing reports of a high standard that are understood by the person/people who they are written for.

The role will be based at Cwmbran House, Mamhilad Park Estate, Pontypool but you will be required to travel to other parts of the Health Boards services, care homes and local authority or other service areas.

Qualifications

Essential

  • Educated to Master's Degree Level or demonstrable equivalent knowledge and experience
  • Evidence of CPD - portfolio of professional development activities.


Desirable

  • An appropriate postgraduate management and/or professional qualification.


Knowledge

Essential

  • Professional knowledge supplemented by speciality training and experience in complex care.
  • Regulation & Inspection Social Care Act (Wales).
  • Safeguarding Adults at Risk Policy and Procedures.


Desirable

  • Knowledge and understanding of primary, secondary and continuing care services
  • Experience of chairing complex meetings with a multi-disciplinary/agency context


Experience

Essential

  • Experience of working in a professional capacity within an NHS setting
  • Safeguarding meetings and undertaking safeguarding investigations.


Desirable

  • Experience of Continuing NHS healthcare assessment process
  • Experience of working in community/primary care services experience of working with the voluntary and or independent sector.
