We are looking for an enthusiastic team player to come and join our senior nursing leadership team as a full time band 6 junior sister. If you have an interest in frailty, sub acute care and rehabilitation and want to work in a high functioning team to support patients and their families during the most challenging of times, this could be the post for you. We are looking for a skilled nurse to take responsibility for the day to day ward management and provide skilled evidence based nursing care.
We are a 28 bedded ward. Patients are cared for by all members of the multi-disciplinary team. The Unit is staffed 24/7, and medical support is provided by Advanced Nurse Practitioners
Successful applicants will be expected to work a variety of shifts over a 7-day period including days and nights and weekends.

Main duties of the job
As a junior sister you will be responsible for providing care to people admitted to our ward for rehabilitation, including training and supervision of junior staff. You will be required to manage the ward in the absence of the ward manager on a day to day basis. We work closely as a multi-disciplinary team. Our focusispromoting the comfort and wellbeing of patients, enablingthemto live as independently as possible. Wyre Forest ward is a small but friendly team to work with. As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice working alongside all members of the MDT as well as patients, their families and carers to provided 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

- Carry out skilled, evidence- based nursing care.
- Demonstrate an analytical approach to clinical assessment of patients in the light of current research and resources.
- Work as a named nurse for a caseload of patients, coordinating the multidisciplinary assessment, planning, delivery and evaluation of care and treatment, ensuring patients have an individualised care plan.

- Work closely with the care coordinator, social worker and all Primary Health Care team members and others concerned, to facilitate safe discharge or transfer of patient care.
- Supervise care given by staff nurses and HCAs, Rehabilitation and other support members of the team.
- Delegate nursing care appropriately to junior staff.
- Contribute in undertaking Staff Development Reviews annually, for junior staff, participate and support clinical supervision and reflective practice within the ward team.
- Adhere to the Trusts policies and procedures.
- Adhere to local and national Health and Safety regulations and ensure that other members of the ward team are aware and adhere.
- Ensure safe cover for the ward within the set staffing levels and to inform the Ward Manager if this cannot be achieved.
- Attend and participate in the hospital management clinical team and other appropriate hospital and staff meetings.
- Identify risk within the ward/department and take appropriate action and to promote the safety of the patients, staff and visitors to the ward.
- Facilitate audit: undertake service audit within the ward/unit
- Support the Ward Manager / Matron/Hospital Manager in managing the ward / service within agreed budget and liaise with Matron/Hospital Manager on budgetary issues.
- Be aware of relative cost of items in common use and ensure efficient use of ward resources.
- Participate in staff recruitment and selection processes.
- Deputise for / represent the Ward Manager / Matron/Hospital Manager in their absence and on a rotational basis hold the hospital bleep if necessary.
