We are pleased to announce an opportunity for a secondment/ fixed term contract for a Band 7 Clinical Discharge lead for to join our UHD Discharge Service, the position will be based at Poole Hospital as part of the system integrated Transfer of Care Hub ( TOCH) . We are looking for a dynamic and autonomous practitioner to work within team linking with internal and external partners.

The Discharge Team is a multidisciplinary team consisting of nurses therapists, and administrative staff working from the acute hospital sites providing expert assistance to the clinical specialties to support effective discharge planning, coordinate referrals to the TOCH.

This team provides complex discharge planning support and guidance , expert liaison with external community service providers, the Local Authorities and Voluntary sector to achieve a safe and timely discharge.

The Team is dynamic, the secondment/fixed term position will offer the individual opportunities to develop enhanced skills in expanding knowledge in complex discharge management and coordination, and enhance their negotiation skills.

This post is an opportunity to be involved in the continued development of the Our Dorset Discharge model from the acute hospital , offer experience in participating in service development work.

Please note: It is essential that applicants have UK NHS experience.

Qualifications

Essential

  • Registered Nurse/Registered health care professional ( AHP) relevant to speciality BSc Nursing or AHP equivalent
  • Evidence of clinical experience in an acute hospital within a leadership/mangerial role


Desirable

  • A masters level qualification


Experience

Essential

  • A proven track record of operational service management at a senior level in an acute hospital including staff management , resource management and change management
  • Ability to review complex information and present this to a wide range of staff in an understandable way
  • A good understanding of the changing NHS environment, and evidence of change mangement


Desirable

  • Clear and demonstrable evidence of advanced, autonomous practice in the management of patients / service users
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training


Knowledge

  • Experience of leadership and influencing skills with the ability to enthuse, motivate and involve individuals and teams
  • Evidence of proactive involvement in the supervision and development of members of the multi-disciplinary team and able to demonstrate changes of practice through these
  • Able to demonstrate wide knowledge of health and social care policy and its application in practice and evidence of developing involvement in influencing local and national standards
