If you are a forward thinking and dynamic clinician (nurse, paramedic, or mental health nurse), on the NMC register, passionate about the physical health of people living with mental ill health across Cambridgeshire, then this will be the role for you!

Our Physical health lead will work collaboratively with all disciplines as part of a multidisciplinary team. Their strength will be in coordinating and delivering our physical health strategy for CAMEO's service user, including the assessment of their physical health, through various clinical settings including the individual's own home environment, where appropriate.Regular monitoring of physical health measures healthy lifestyle advice and access to physical health interventions will be paramount in the deliverance of this service.Our PH lead will have a keen interest and focus on Early Intervention in Psychosis (EIP) National Standard for physical health care for our client group, and a willingness to shape our service offer.Liaising with GPs and other healthcare professionals to deliver a holistic approach to care, plus keeping abreast of health care records and updates, are vital in ensuring our audit standards are met.

Main duties of the job

To provide professional leadership for staff within the Cameo service on service user assessment and care planning of physical health and wellbeing.

To audit and monitor physical health assessments within the team, and in doing so, ensuring the service adheres to the NICE National Early Intervention Standards target of 90% of service users having received or refused a physical health assessment. Close monitoring of DNA rates to be part of this.To have sound knowledge of IT systems and be proficient in the use of these ensuring they meet the data requirements for Cameo in relation to physical health checks.To have good evidence-based knowledge of the physical health needs of Early Intervention Service clients.To directly supervise the band 5 Cameo physical health coordinator post and ensure consistent practice across the whole of the Cameo service.

Assessment and intervention to be delivered via a health coaching model in relation to physical health, namely effective self management of any physical health illnesses, weight management, nutritional advise , substance misuse, sexual health, exercise regimes, smoking cessation.

To lead, deliver and oversee the smooth running of CAMEO trust wide physical health clinic ensuring :

Preparing for physical health assessments by thoroughly reading all previous physical health information on the Trusts electronic records

Completing a thorough and meaningful physical health assessment for each client that attends the clinic or is being seen at home which includes: Discussing current medication and side effectsDiscussing any ongoing or recently established physical health diagnosesDiscussing family history of physical health conditions and use this to inform assessmentComplete physical health measures: blood pressure and weight/BMIComplete blood tests, measuring cholesterol/TG, glucose (fasting/random or HbA1c)Complete ECG readings in the clinicRefer for MRI scanDiscuss smoking habits, illicit substance misuse and alcohol useDiscuss diet and exerciseDiscuss sexual healthAssess for fallsProviding physical health advice service users can take to improve health, including diet and exerciseReferring/signposting to relevant sporting and exercise servicesReferring/signposting to relevant substance misuse servicesReferring to dietician and other health servicesReferring/signposting to sexual health servicesReferring to Physio / occupational therapy as requiredResponsibility for ensuring that all necessary follow up is completedLiaising with care coordinators regarding physical health needs of their clients and the follow up required

Liaising with medical staff around complex physical health issues and blood and ECG resultsContacting clients regarding any necessary follow upLiaising with GPs around physical health findings. This includes writing an outcome letter to each GPEnsuring that the physical health clinic is clean and hygienic

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. Education /Qualifications

Essential

  • Registered Mental Health Nurse (RMN)
  • General Nurse (RGN) with an interest in mental health
  • Evidence of further training appropriate to physical health management and interventions or willingness to undertake


Desirable

  • Evidence of continued training and professional development
  • Evidence of training appropriate to adult mental health care.
  • Evidence of training appropriate to physical health management and interventions
  • Health coaching qualification.


Experience

Essential

  • Experience of working with adults with psychosis.
  • Experience of working independently.
  • Experience in undertaking physical health checks.


Desirable

  • Evidence of continuing professional practice and development.
  • Experience of managing workload and ability to prioritise
  • Experience in running groups
  • Evidence of clinical supervision experience
  • Experience of working with individuals, families, multidisciplinary teams and community services.
  • Lived experience of mental health


Skills and Abilities

Essential

  • Competent in undertaking a range of physical health ? Experience with working with adult service users, their checks including blood taking, ECG, BP, sexual health screening
  • Ability to work autonomously
  • Knowledge base around suitable interventions to address identified health issues.
  • Able to develop good therapeutic relationships with clients
  • Planning and organisational skills
  • Confident and competent in delivering educational sessions.
  • Skills in self-management including time management
  • Able and confident to communicate well with a variety of people including GP's , practice nurses, health specialists and other key stakeholders.


Desirable

  • Experience with working with adult service users, their families and carers


Knowledge & Understanding

Essential

  • Knowledge pertaining to physical health checks and baseline measures and knowing when further action is indicated.
  • Knowledge around addressing identified health issues and of community resources and teams to partner with.
  • Knowledge of contemporary policy and legislation relating to mental health and social care
  • Ability to identify and employ, as appropriate clinical governance mechanisms for the support and maintenance of effective clinical practice
