We are looking for a motivated and enthusiastic Band 7 OT to work as part of the Sutton Health and Care Reablement Unit to provide an inpatient rehabilitation service for service users within South West London. Post holders will be working as part of an integrated multidisciplinary team including nurses, occupational therapists, physiotherapists, social workers, rehabilitation assistants, healthcare assistants and administrators, as well as the voluntary sector. The post holder will be based on the inpatient reablement unit and provide a rehabilitation over a 7 day period.
Main duties of the job
To work as a key member of the multidisciplinary team in the assessment and management of a caseload of clients on the reablement unit within timeframes set.
To plan, deliver and evaluate the treatment needs of clients with a variety of complex social and health care needs.
To participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population.
To promote effective teamwork within SWL and Sutton Health and Care, delegating activities appropriately, to the benefit of the clients and service delivery.
The expectations within this job description will be achieved through hands-on clinical practice, education and training and research.

Essential
- Occupational Therapy diploma / degree
- Current registration with HCPC
- Evidence of post grad training (leadership, frailty, rehab)
- Evidence of ongoing CPD in appropriate areas
- Valid UK driving licence
Desirable
- Membership of the RCOT.
- Evidence of clinical educator's course
- Use of car for work
Experience
Essential
- 2 years experience to include community/rehab/frailty/discharge to assess
- Evidence of service performance projects
- Evidence of teaching students, peers and junior staff
- Evidence of autonomous working
- Evidence clinical governance / audit / clinical effectiveness projects
