An exciting opportunity has arisen for a dynamic Nurse to join the amazing Poole Intermediate Care Service. We are a multidisciplinary Team who prevent hospital admissions and secure early hospital discharges.
We deliver a hospital at home service, caring for patients in the comfort of their own homes.
This is a part time post of 22.5 hours per week. The post holder may berequired to work flexibly to meet the needs of the service.
Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.
Main duties of the job
To demonstrate leadership.
To provide a high standard of holistic assessment and appropriate timely reviews.
To plan specialist care and/or rehabilitation programme, to promote and sustain independence/well-being for the individual in their own homes.
To deliver Nursing/Rehabilitation/care programmes for adults, many of whom have complex clinical and social needs with the aim of improving independence, function and quality of life.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

- Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
