Hammersmith & Fulham Health Visiting - Health Visitor - Band 6
Within Hammersmith and Fulham (H& F) CLCH provide a range of services including Health Visiting and School Nursing, Children's Therapies as well as numerous adult services, this means that you will benefit from a close working relationship with peers and a joined-up approach to care leading to improved health outcomes for the people you care for.
Within your team you will benefit from a Borough lead with a wealth of experience in Children's health, Safeguarding, School Nursing and Immunisations and a team that has in excess of 300 years' experience who are ready to support your learning and development. The team in Hammersmith are based within Parkview Centre for Health and Wellbeing, Richford Gate, Park Medical centre, Parsons Green Health Centre.
There is shared vision between commissioner and LCH to align Health Visiting with family hubs, this is will families to access a range of services at once and facilitate a supportive peer network for staff across services.
For registered Band 6 Health Visitors, this post is offering a premium of �2,500 payable upon commencement of employment (T& C's apply)

Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required Utilising specialist skills and knowledge, advise and support individuals and families on evidence-based health care practices. Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs. Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population
