The Tissue Viability Support nurse will assist and work independently within the Tissue Viability Team. To actively contribute to the prevent ion of avoidable pressure ulcers and involved in the management of complex wounds and leg ulcers within either clinic or community setting. To deliver training formally or informally on all aspects of Tissue Viability to patients, carers and to all staff groups and multidisciplinary team. To act as a specialist practitioner and expert resource for patients with tissue viability issue, their families, carers and other healthcare professionals within the wider multi-disciplinary team.

Main duties of the job

CLINICAL
  • To provide specialist advice of best practice relating to all aspects of Tissue Viability in line with both local and national guidance to all staffs, patients and carers.
  • To ensure safe practice within the NMC Code of Professional Conduct and adhere to the trust's policies and procedures.
  • To actively contribute in the prevention of all pressure ulcers and provide specialist advice to all staff, patients and carers and other healthcare professionals.
  • To establish and maintain effective relationships with patients, carers and the wider multi-disciplinary team in order to facilitate patient choice and independence.
  • Assess, plan, implement and evaluate clinical care holistically.
  • Actively support innovations and improvements in practice related to wound care including participation in the assessment and evaluation of wound care products and equipment with the aim of providing cost effective care.
  • To ensure personal, professional, and clinical expertise is upheld through regular evaluation and updating of individual practice.

Essential and Desirable

Essential

  • oEducated to Masters Level in relevant clinical speciality or equivalent combination of post graduate diploma, courses and experience
  • oRegisted Nurse
  • oExtensive experience as a tissue viability nurse
  • oPost registration course in wound management or leg ulcer management
  • oIndependent non-medical prescriber or willingness to undertake.
  • Post graduate/Masters level
  • oOrganisational skills
  • oCare Planning and caseload management
  • oWritten and verbal communication
  • oAbility to work as part of a team
  • oAbility to work as a autonomous practitioner oUse own initiative
  • oAble to support junior staff and students
  • oAssessments skills
  • oAdvanced wound technologies e.g. VAC therapy, intermittent pneumatic compression, bio-surgical debridement
  • oHealth Promotion
  • oExcellent interpersonal skills
  • oAbility to teach clients and colleagues, both formally and informally
  • oAbility to work across boundaries to develop new ways of service delivery
  • oPresentations/ Report writing
