As a Registered Nurse you provide direct personal care to service users/patients, without direct supervision, and co-ordinate the care given bycolleagues.The post holder will deliver high quality safe nursing care, ensuringconsistently high standards of practice and clinical excellence which complywith the Nursing and Midwifery Council (NMC) code of professional conduct.The role includes leading the delivery of comprehensive service userassessment, planning, implementation and evaluation of person-centeredcare in a collaborative way. The post holder will co-ordinate a first classservice that includes assessment and monitoring of physical health,maintaining service users rights, supervising junior staff and students, working collaboratively with the Multi-Disciplinary Team and maintaining a safe and therapeutic working environment and accurate records.Band 5 nursing roles are only open only to staff with a nursing registration,obtained as a result of completing a qualification recognised by the Nursingand Midwifery Council (NMC).

Job responsibilities

• Provide high-quality nursing care:Work autonomously to deliver physical, psychological, social, recreational, and spiritual care. Use therapeutic engagement and activities to promote recovery. Address any arising issues.
• Ensure basic needs are met:Oversee activities for nutrition, hydration, hygiene, comfort, emotional, and social support.
• Coordinate patient-centered care:Collaborate with the multi-disciplinary team (MDT) on care plans, get feedback, and improve them.
• Communicate effectively:Provide clear information about care to patients and families.
• Teamwork and collaboration:Work positively with the MDT and partner organizations.
• Medication management:Administer medication, monitor side effects, and ensure patient compliance.
• Maintain a safe environment:Ensure safe medication storage, sharps disposal, and clinical equipment use. Report incidents per trust policy.
• Care plan development and monitoring:Collect data, track patient progress, discuss findings, and generate reports. Develop and execute care plans under Registered Nurse supervision.
• Lead junior staff:Guide junior staff in providing hands-on care, including patient engagement, patient respect and dignity, care plan creation, monitoring vital signs, promoting healthy lifestyles, and recovery-focused activities.
• Facilitate admissions and discharges:Ensure patient and service needs are met, including required documentation.
• Patient advocacy:Champion patient needs and rights within trust policy, empower patients to manage their care.
