An opportunity has arisen for an enthusiastic and motivated Band 7 Clinical Nurse Lead within the East Kent Community Rehabilitation Service. The successful applicant will be a Senior Clinician who will support the Service Manager and Operational Lead to deliver a high quality, safe and effective service. The applicant will need to demonstrate clear clinical leadership skills, and knowledge relevant to the post. You will need excellent communication and organisational skills and be a team.

The Community Rehabilitation Service operates in five localities across East Kent to provide specialist rehabilitation with service users in their own homes, in which to enable individuals with complex and enduring mental health needs to manage more effectively in the community. Each locality has a dedicated multi-professional mental health team who work collaboratively with Third Sector agencies providing additional KCC Social Care and landlord / tenancy support in a Partnership known as Horizons.

It is highly recommended that you arrange an informal visit in order that you can gain an insight into the Community Rehabilitation (Horizons) Service.

Main duties of the job

  • Provide clinical and quality nursing leadership across the service
  • Be flexible and responsive according to the needs of the service

  • Demonstrate good mental health assessment and intervention skills.
  • Use working knowledge of good practice to support audits e.g. CliQ checks
  • Provide coaching and mentoring to nursing colleagues
  • There is a strong commitment to supervision, training and regular appraisal, which you will receive and deliver to the team
  • Coordinate patient safety and safeguarding across the service
  • Manage the referral pathway
  • Support the Service Manager and Operational Lead in service development
  • Ensure compliance with physical health monitoring and KMPT clinical standards
  • Facilitates preparedness for NMC Revalidation and subsequent meetings in the Role of Confirmer.
  • Foster and develop working relationships across KMPT; CMHT's, EIPS, Psychiatric Liaison Service, Acute Wards, inpatient Rehab etc.

Job description Job responsibilities

  • Provide clinical leadership to a team of clinical staff at band 6 and below.
  • Has a clinical leadership presence and is able to demonstrate clinical expertise.
  • Is an integral part of the Service Senior Management team.
  • Shares the Rehabilitation Service vision and goals to ensure team objectives are communicated and actioned.
  • Creates effective communication across the teams, ensuring staff are both kept up to date with Trust developments and information from the team is communicated with the Service and Operational Manager.
  • Promotes a culture of equality and diversity.
  • Works with the 5 locality teams and their team members to engage and motivate them.
  • Supports the clinicians and their clinical work in a responsive and timely way.
  • Supports the Service Manager and contributes to the portfolio of the management team.

  • Acts as a role model and mentor
  • Maintains accountability and integrity in line with the NMC Codes Professional Standards of Practice and Behaviour for Nurses.
  • Contributes to recruitment, selection and retention activities in order to build a diverse team with the knowledge, skills and experience to deliver a high-quality service.

  • Provides clinical supervision to Nursing staff within the Service.
  • Provides feedback, support and mentoring in relation to the clinical quality of their work.

  • Leads the delivery of a timely, safe and effective service
  • Keeps up to date with KMPT Policies and ensures the team are aware of their responsibilities according to Professional Code and Trust Policy and legislation
  • Implements Trust policies and local protocols and monitors the teams adherence in clinical risk assessment and management.
  • Recognises how diversity, culture, beliefs and values may influence decision making
  • Ensures the team adhere to theKMPT and KCC Safeguarding Policy
  • Ensures clinical record keeping standards are compliant with KMPT Standards; that information is clearly communicated and includes a management plan where appropriate.
  • Ensures the team maintain and improve on quality standards.
  • Seeks and encourages feedback from patients, carers and referrers to improve the patient experience e.g. promotes PREM
  • Identifies risk and escalates when appropriate.
  • Understands the impact of change management.

  • Is aware of the purpose of the Service, and supports the team to meet its objectives.
  • Ensures team members have a clear understanding of policies and procedures.
  • Contributes to the fundamental standards of quality and safety underpinned by the CQC to ensure the best possible care for patients, service users and their family and friends.

Person Specification Qualifications & Training Essential
  • Registered Mental Health Nurse
  • Further training relevent to the role at the level of post graduate MSc or equivilent
Experience Essential
  • Previous experience at Band 7 or above
  • Minimum of 3 years diverse post qualification experience
  • Extensive clinical experience in group and individual work
  • Supervision experience

  • experience of audit and clinical governanceprocedures
Knowledge & Skills Essential
  • Advanced understanding of mental health problems, treatment options and models appropriate to rehabilitation and recovery

  • Detailed knowledge of relevent legislation, polices and procedures relating to MHA, MCA, CPA
  • Experience in managing quality standards
