The North East Ambulance Service is offering a dynamic opportunity for a dedicated individual to join our forward-thinking Patient Safety Team as a Patient Safety Investigation Lead.

In this pivotal role, you'll work within our Quality and Safety directorate, collaborating closely with our Learning Response Leads. You'll be at the forefront of patient safety, taking charge of monitoring incidents, supporting lead investigators, and ensuring we uphold the highest standards of care.
  • Supporting the implementation of the National Patient Safety Incident Response Framework.
  • Oversee and ensure compliance with the duty of candour, addressing any emerging safety themes.
  • Drive rapid learning and improvement by working closely with service lines to ensure lessons from incidents are effectively shared across the organisation.
  • Generate detailed reports and present your findings at key governance and safety forums, making your voice heard at the highest levels.

Main duties of the job

To work within a team with responsibility for the oversight of all Patient Safety Incidents (PSIs) activity, including the analysis and formulation of learning outcomes for all PSIs reported under the Trusts Patient Safety Incident Response Framework (PSIRF) policy and plan.

This role will coordinate and identify learning from patient safety investigations, complaints & concerns to ensure effective mechanisms are in place for sharing these lessons learned both internally and externally to the Organisation as appropriate.

The post holder will ensure the completion of Learning From Patient Safety Events (LFPSE) records. They will be responsible for the oversight of monitoring trends in reported patient safety incidents and concerns.

The post holder will have a visible presence in the Trust representing the Patient Safety Team and be responsible for leading effective, timely and proportionate responses to patient safety incidents, by working collaboratively and proactively with NEAS staff in accordance with Trust, National and Commissioner policies and good practice.

The post holder will work in line with Trust's Patient Safety Incident Response Plan and Policy and be responsible for leading proportionate responses to incidents.

Person Specification

Education, Qualifications and Training

Essential

  • Significant experience in human factors/PSIRF
  • Educated to degree level in relevant subject
  • Evidence of continuous professional development at the equivalent of post graduate level.
  • Teaching/mentorship qualification or demonstrable experience.
  • Accredited Leadership programme or willing to work towards.


Desirable

  • Experience in a patient safety role within a healthcare setting.
  • Further qualification in statistical analysis, quality improvement or clinical background.
  • oCompleted an accredited PSIRF training or equivalent knowledge gained through experience and willing to train
  • Management qualification.


Skills and Knowledge

Essential

  • Knowledge of the Health Service and current national initiatives, particularly in relation to Clinical Governance and Patient Safety.
  • Clear understanding of the issues prevailing around patient safety.
  • Knowledge of LFPSE reporting requirements.
  • Knowledge of CNST/ NHSLA requirements.
  • Knowledge of patient safety incident management and LFPSE reporting.
  • Excellent communication, presentational, training and report writing skills
  • Can prioritise and work to tight deadlines to deliver a broad range of complex activities whilst being mindful of adjustments that may be needed due to uncertainty
  • Ability to influence/persuade/motivate others
  • Skills required to undertake evaluations and/or audit
  • Excellent communication skills both written and verbal
  • Analytical skills and ability to analyse complex data
  • Report writing skills for board level meetings
  • IT skills using Microsoft office applications such as Excel, Word and PowerPoint

  • Knowledge or experience of NHS clinical risk systems e.g. Ulysses or Datix


Experience

Essential

  • Experience of contributing to CNST/ NHSLA requirements
  • Experience of advising and liaising with staff at all levels, preferably in the NHS
  • Experience of writing policies within a NHS environment.
  • Ability to write professional reports to a high standard which require consolidating complex information from a variety of sources


Desirable

  • Experience of working in a complex NHS environment in a governance or patient safety related role.
  • Experience of successfully leading change management projects involving multi-disciplinary team
  • Experience of using PSIRF human factors toolkit for investigations.
