An exciting opportunity for the post of Social Worker has become available within North Hertfordshire Adult Mental Health Services.
You will have experience of working in a Community based setting. You will be responsible for the delivery of care to an allocated caseload of clients living in the Community experiencing, or recovering from, mental health difficulties.
Main duties of the job
You will provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control. You will ensure that appropriate health and social care services are commissioned as required. You will also assess, identify and evaluate Client's needs through comprehensive assessments, reviews, care planning and CPA in conjunction with other professionals.
You will work as part of a multi-disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust.
You will also carry out AMHP duties or be willing to undertake AMHP Training.
The postholder will be required to demonstrate their knowledge of the relevant legislation, theory and policy in relation to the care group.

Person Specification KNOWLEDGE, TRAINING AND EXPERIENCE Essential
- An appropriate professional social work qualification (e.g. DipSW, )
- Education to degree level
- Registration with General Social Care Council
- Training in principles and application of outcomes focussed assessments and personalisation
- Knowledge of legislation, theory and policy in relation to relevant care group
- Experience of undertaking assessments, focussed as personalisation, choice and control, and care planning
- Knowledge and understanding of personal budgets and direct payments
- Experience of multidisciplinary working and ability to work collaborative
- Experience of partnership working with service users and carers, demonstrating dignity and respect
