This is an exciting opportunity to work in a service committed to improving the heath and wellbeing of frail and or older people.

The Community Assessment and Treatment Services (CATS) provides rapid access to doctors, nurses, physiotherapists and OTs for primarily frail and/or elderly patients with a strong focus on prevention of hospital admission.

We are looking for an Occupational Therapist to support Marlow and Thame CATS as well as across the Buckinghamshire community within clients homescarrying out occupational therapy assessments and treatment.

Main duties of the job

The successful candidate will spend part of their week in the Marlow and the Thame clinic liaising with the multidisciplinary team. Most of their time will be spent in the community. Patients who have seen the doctor, nurse and physiotherapist in clinic are then seen by the occupational therapist within the patient's home.

The services run from 08:30 to 17:00 Monday to Friday excluding bank holidays.

Due to the nature of the role it is essential that applicants are able to travel effectively and independently on a daily basis to and from the base, clients' homes and other venues around the geographical area. If necessary, adjustments can be considered, in accordance with the Disability Discrimination Act.

Person Specification

Education, Qualifications and Training

Essential

  • BSc in occupational therapy
  • Health Professions Council Registered
  • Evidence of post registration Continuing Professional Development portfolio


Desirable

  • Relevant post graduate clinical training e.g. Elderly, LTC s, Cardiac Disease, Dementia, Rehabilitation, Trauma and Orthopaedics or similar


Experience

Essential

  • A minimum of 2 years post graduate relevant clinical experience
  • Experience of managing a case load of older people
  • Experience of working within a community setting
  • Experience of supervision and training of junior staff and assistants


Desirable

  • Experience of working within multi agency / multi disciplinary team
  • Experience of electronic clinical systems
  • Experience of assisting with service developmen
  • Experience participating in a research or audit project


Skills, Abilities and Knowledge

Essential

  • Knowledge and ability to provide patient assessment, treatment planning and goal setting in varied conditions
  • Clinical reasoning skills relevant to the client group (frail/ complex conditions/ fallers).
  • Ability to work both independently and as a member of a team including working across professional boundaries
  • Knowledge of best practise within the specialist area
  • Excellent written and verbal communication skills
  • Good problem solving abilities

  • Report writing skills and IT skills
  • Presentation skills
  • Ability and willing to learn new skills and competencies
  • Ability to promote health
