To work closely with all members of the multi professional team to ensure that Patients receive seamless care within the Trust and community. To provide specialist advice to support assessment, planning and implementation of care, and where appropriate to deliver programmes of care. To carry out specialist Nursing procedures. To contribute to specialist education and training programmes for other staff and learners.
Main duties of the job
Establish and maintain effective communication with stressful topics in a range of situations.
Develop knowledge and practice in an area of work
Monitor and maintain health, safety and security of self and others in own work area
Contribute to the development of services
Contribute to quality improvement.
Promote people's equality, diversity and rights
Assess and address people's health and wellbeing needs when those needs are complex and change across the caseload
Develop practice in addressing individuals health and wellbeing needs
Lead others in the development of knowledge, ideas and work practice
Research and development
Day to day roles include but are not limited to dealing with IBD helpline calls/emails giving general advice, requesting required tests and possibly admitting patients to hospital, follow up clinics (when competent), checking results of bloods and other tests, pre assessing patients for biological therapies, prescribing and ordering treatments, ensuring documentation is prepared for weekly MDT meetings and completing agreed outcomes following this meeting, keeping service documentation up to date, monitoring IBD inpatients progress, registering patients for new treatments once all screening tests complete, keeping on top of homecare prescriptions for homecare patients, tracking endoscopy patients results for suspected and current IBD patients, attending meetings necessary for our service.
