The Community Rehabilitation and Falls Service (CRAFS) is recruiting a highly motivated and enthusiastic Band 7 Community Occupational Therapist to join our supportive team.

Our Lambeth OT service is well- established and you will be joining at an exciting time when we have new funding to set up an OT service in Southwark, for which your role will be instrumental. It is an excellent opportunity for a Band 7 OT with an interest in community work or an experienced Band 6 OT looking for a more senior role.

You should have experience of delivering high quality rehab to older people with a variety of complex conditions and falls. You will provide the leadership and day-to-day coordination of our community occupational therapy service and will work with a multi-disciplinary team to implement person-centred rehabilitation in the community.

The post offers opportunities to develop leadership skills by enabling you to take a key role in a number of exciting service developments. CRAFS has close links with occupational therapy colleagues in other community services and the hospitals.

Excellent communication skills and an ability to prioritise a caseload of complex patients alongside the role of team lead are essential. The successful candidate will demonstrate flexibility, creativity and the ability to support change in a fast paced environment. The post holder will also contribute to the 6-day intermediate care therapy service on the Pulross Centre inpatient ward in Brixton.

Main duties of the job

  • To perform highly specialised and advanced assessments and treatments as part of the Community Rehab and Falls Service, providing home-based, outpatient and intermediate ward-based therapy to adults living in Southwark and Lambeth. The caseload consists of adults and older people with a wide range of complex pathologies, functional difficulties and disabilities.
  • To act as team leader, manage designated staff performance and be responsible for the day-to-day co-ordination of the Occupational Therapy service as part of the Community Rehabilitation and Falls Service. The post holder will provide clinical leadership and a source of clinical expertise to facilitate seamless rehabilitation of clients.Provide supervision and facilitate professional development with Occupational Therapy staff, Therapy Assistants and Occupational Therapy students as required.
  • To work autonomously, frequently as a lone practitioner and at times in conjunction with team colleagues, across a variety of locations within the community including client's homes and residential care settings to meet the individual needs of the multi-cultural population in an inner-city area.
  • To work across borough boundaries, accessing and recording information, communicating with patients and colleagues, attending meetings or training using mobile devices, and coming into offices when required.

Qualification and education

Essential

  • oHealth and Care Professions Council registration as an OT


Desirable

  • oMSc in Occupational Therapy


Experience

Essential

  • Significant rehabilitation experience at specialist level across relevant clinical areas, e.g. elderly care, intermediate care, dementia, orthopaedics, falls
  • Experience of managing less senior staff and students including providing supervision and appraisal
  • Experience working in falls prevention and management


Desirable

  • Working in a community setting
  • oExperience of research


Knowledge

Essential

  • Knowledge of the rehabilitation of older people, including the frail elderly
  • Specialist knowledge base relevant to service area gained through work experience and formal post-graduate training at or equivalent to Master's level
  • Highly specialist knowledge of a range of assessment and treatments relevant to the client group.
  • Knowledge and understanding of how the ageing process impacts on health.
  • Knowledge and experience of quality improvement methodology
  • oUnderstanding of how clinical Governance relates to Occupational Therapy practice.


Abilities

Essential

  • Ability to carry out risk assessments, including those for moving and handling, in a variety of settings
  • Plan, implement, and evaluate treatment programmes effectively in physical, cognitive and perceptual rehabilitation
  • oAble to assess complex posture and seating needs and provide appropriate recommendations.
  • oTo assess equipment provision and minor home modifications to facilitate complex discharge
  • oProficient IT skills. e.g. word processing, email, PowerPoint, excel
  • oAbility to manage a complex and constantly changing workload of clinical and non- clinical duties demonstrating excellent time management, prioritisation and organisational skills
  • highly developed verbal, non-verbal and written communication skills in English.
  • oAbility to demonstrate skills sharing and collaborative working at a variety of levels
  • oThe ability to devise, deliver and evaluate training both within the service and to other agencies.
  • oAble to work as part of a multi-professional team, understanding the roles of others, both within and external to the Trust.
  • oAbility to adopt innovative and flexible working practices in order to integrate interventions into the client's home environment.
