We are recruiting for a End of Life Care Nursewho shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
This dynamic and compassionate role involves providing expert holistic care and support to patients approaching end of life and their families. As an End of Life Care Nurse, you will play a crucial role in providing compassionate and holistic care to patients in their last hours to days of life.
This position involves working within a multidisciplinary team to ensure that dying patients receive the best possible care tailored to their individual needs and preferences. You will be expected to work autonomously as well as part of a multidisciplinary team. This will involve planning and managing individuals with complex physical and psychosocial needs. We aim to give the highest quality care that aligns with patient preferences and respects individual dignity.
Main duties of the job
See all adult patients across the trust who require end of life care as part of a 7-day service
Undertake patient assessments and plan individualised care for patients at their end of life and deliver direct patient care when needed
Deliver direct patient care as required
Deliver and implement patient, family and carer support- being the person's advocate Advocate for the patient and their family/carers/ Liaise with the wider multidisciplinary team including community services
Deliver the EOLC trust wide educational programme
To assist with quality improvement projects within the service
To ensure the development, dissemination and implementation of best practise in End of Life Care in accordance with NICE guidance
To undertake End of Life Care Audits- NACEL and Dandelion Care Bundle Audit
To assist the implementation of EOLC improvement strategies using Patient First methodology.

The End of Life Care Team works in collaboration with the Palliative Care Team allowing for a seamless transition through the patients individualised care journey.
We believe that 'Every Person Counts' and in believing this out ethos is to build/plan and deliver care based on the person's needs and wishes.
Currently the service covers Monday- Friday time period with the introduction of a new valued team member the service will evolve into a 7 day service
To act as a point of reference regarding end of life care advice for all nursing, pharmacy and medical staff within the hospital
To proactively assess, plan, implement and evaluate patients based on an agreed plan of care that support quality and timely care, in line with their needs and are recorded accordingly on the trust Electronic Person Record system (EPR)To ensure that the patients 'physical, emotional, psychological, social, and spiritual needs are addressed and regularly reviewed
To ensure quality standards are maintained and re-evaluated, to provide the highest standards of care that reflect Medway NHS Foundation Trust policies and procedures
Anticipate and respond to patient/family concerns
Be able to work independently as a lone worker
Be accountable for the administration of drugs and the maintenance of records and to maintain up to date knowledge of the purpose, actions and side effects of drugs
Expertly manage complex symptoms through pharmacological and non-pharmacological interventions, promoting comfort and quality of life.

- Registered Nurse- Adult
- Educated to level 6 (degree) or equivalent level of education, training or experience
- 3 Year post registration
- Excellent Communication skills
- Passionate about end of life care
Desirable
- Mentorship course or equivalent
- Clinical experience in End of Life Care- and the guidance that governs practice
- CQC fundamental standards applicable to end of life care
- Competent clinical skills- Patient assessment, symptom assessment and symptom management
