The post is based within our home treatment team in New Horizons.
The home treatment team`s support patients in their homes to prevent hospital admission. The role is predominantly Monday to Friday.
The ideal candidate will be someone who has worked in an acute health care setting at Band 6 and feels ready for a next level challenge.
They need to be motivated to work in a high demand and clinically exciting service. The ideal candidate will be someone who truly values the opportunities that the mental health professionals have in assisting people going through difficult times in their lives. They will be passionate about helping people with SMI getting back on their recovery journeys following relapses.
The team is very supportive with many very experienced practitioners from different clinical backgrounds.

Main duties of the job
- Treating patient`s in their homes in order to prevent hospital admission.
- Carrying out reviews for patients referred to the crisis resolution home treatment team.
- Involved in the delivery of care of the patients taken under the care of CRHTT
- Attendance to referral meetings Take part in complaints and serious incident investigations
- Liaising with other parts of the mental health service
- Assessing and supporting patients both via home visits and phone/video calls
- Participating in the QMIS process for service improvement
- Supervising junior staff and students
