The Practice Nurse is a key member of the Primary Care Team and will be expected to provide comprehensive primary care services to registered patients and work effectively with all colleagues within the team to deliver high quality patient centred care.
An autonomous Practitioner responsible for assessing, implementing, developing and evaluating programmes of care for patients with chronic disease, minor acute illnesses and contraception, in line with relevant knowledge and skills.
To provide nursing expertise and leadership to the wider primary health care team
Main duties of the job
Assessing, implementing, developing and evaluating programmes of care
Clinical risk assessments and formulations of management plans
Supervision and teaching for other staff
Immunisations and vaccinations
Assist in developing local guidance and protocols
Clinical assessments, including history taking, and providing care for those with a range of illnesses/ conditions
