The secondary breast clinical nurse specialist role provides specialist nursing advice and support to patients, diagnosed with a secondary breast cancer, and their relatives and carers.
The post holder will facilitate the development of the secondary breast cancer pathway.
Work collaboratively with the multidisciplinary teams within the breast unit and the oncology unit across the Trust.
Work closely with the ward staff and the wider multidisciplinary team members.
Liaise with primary and secondary team members to facilitate and co-ordinate patient centred care.
Ensure patients receive all the relevant information to enable them to make an informed choice about their treatment.
Work with the patients, families and carers to assess patients needs (using the holistic needs assessment tool), developing individual care plans and to re-evaluate as the ongoing care needs change.
Main duties of the job
o To work as part of the multidisciplinary team within the breast care service and oncology.o As a key worker to the patient you will act as their advocate in co-ordinating their care and treatment plan.o Provide an interface between patients/carers and the multi-disciplinary team, developing nursing practices and identifying opportunities to improve quality of patient care/experience.o Co-ordinate and support clinics and prioritise all referrals from MDT. Leading in the assessment, planning, implementation and evaluation of care.o To identify and refer breast patients for psychological/physio/dietetic support as required.o Exercise leadership and inspire colleagues to improve standards and quality to develop professional practice.o To identify strategies to help inform and educate the patient and family members and support them in decision making, in line with personalised care.o To participate in the development of the individual care pathway for the patient with secondary breast disease.o Undertake and facilitate nursing research and audit reporting findings to the multi professional team.o Work with the breast care team in the implementation of local and national nursing strategy associated with breast cancer.
