An exciting opportunity has arisen for a band 6 specialist nurse role in the renal outpatient team.

Candidates should be committed to the provision of high quality patient care, be highly motivated, dynamic, enthusiastic and demonstrate excellent communication skills

Renal experience is essential as the role will involve supporting the pre dialysis service.

Main duties of the job

  • Provide education to patients with chronic kidney disease on the various forms of renal replacement treatment. The treatment options include haemodialysis, peritoneal dialysis, transplant, or conservative management. A good understanding of these is essential. Education is provided to patients in hospital and also at the dialysis unit off site.
  • Help organise education days for patients and raise awareness of CKD at other relevant events
  • On-going surveillance of pre-dialysis patients, in-putting data and documentation
  • Assist with a current on-going research study.
  • Assist in the renal vaccination clinic.

Applicants must drive and have access to a car for work as will be required to make home visits and home assessment for those patients choosing a home therapy.

Job responsibilities

The Clinical Nurse Specialist will act as the clinical expert and professional resource to the multi- professional team, patients and carers within their Speciality Service.

The post holder will have a responsibility to develop the specialist area of practice in partnership with other members of the speciality team will work collaboratively to develop services and improve the quality of care delivered.

The post holder will carry responsibility for either a defined case load or offering highly specialist service advise this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care, management and education.

The post holder will contribute to achieving the objectives of the clinical service group / speciality corporate objectives and work within the clinical governance framework.

This will be achieved through the core dimensions of the role, clinical practice, leadership, acting as the patient advocate and through audit and education.
