We are offering a fantastic opportunity for motivated, enthusiastic and highly skilled HCPC Registered Occupational Therapist to join our Stone Rehabilitation Team on a 12-month fixed term basis to cover maternity leave.

You will work autonomously but as part of the multidisciplinary team made up of Occupational Therapy, Physiotherapy, Rehabilitation Technicians and Administrators that provide a community rehabilitation service to housebound adults in the Stone and Eccleshall area with a range of physical conditions.

Referrals to the service may include progressive neurological conditions, medical, orthopaedic, falls, cancers, multi-pathology and dementia. The service aims to maintain people at home, prevent hospital admission and promote condition self-management.

Our team is dedicated in delivering excellent customer service and aims to improve function, independence and promote self-management through therapy programmes and/or equipment provision.

Due to the nature of the role it is essential for this post that you have suitable ability to travel independently across a large geographical area within Stone and Eccleshall.

Main duties of the job

As an Occupational Therapist in our service, you will undertake comprehensive assessments using verbal and non-verbal communication, skilled physical assessment techniques and using reasoning to agree therapeutic goals and the treatment programme with the patient/carer. This could be in patients' home, rehabilitation centre or in care homes.

As part of this role you will be expected to:
  • Be clinically responsible for patients accepted onto the caseload and to organise this with regard to clinical priorities, evidence-based practise, ensuring the most efficient use of skills, time and resources.
  • Delegate, supervise and modify treatment programmes to rehabilitation technicians' and oversee their contribution to the patient's goals and outcomes.
  • Work with team members in an integrated manner and promote and develop closer working with the integrated local neighbourhood team in Stone.
  • Have a working knowledge of a variety of Occupational Therapy rehabilitation techniques, including both adaptive and restorative.

Job responsibilities

Stone rehabilitation service is a multidisciplinary team providing community rehabilitation to adults in the Stone and Eccleshall locality.

The team comprises Occupational Therapists, Physiotherapists, Rehabilitation Technicians and Admin support.

Main Duties and responsibilities.

1. To work to College of Occupational therapy and HCPC code of professional conduct and guidelines, having a good working knowledge of national and local standards.

2. To establish and maintain effective working relationships both within the Partnership Trust and with external organisations to meet service requirements and support and demonstrate the Trusts equality values.

3. To be clinically responsible for patients accepted onto the caseload and to organise this with regard to clinical priorities, evidence-based practise, ensuring the most efficient use of skills, time and resources. To work autonomously without direct supervision

4. To undertake comprehensive assessments using verbal and non-verbal communication, skilled physical assessment techniques using reasoning and agreeing therapeutic goals and treatment programme with the patient/carer. This could be in patients home, rehabilitation centre or in care homes.

5. To delegate, supervise and modify treatment programmes to Rehabilitation Technicians and oversee their contribution to the patients goals and outcomes.

6. To work with team members in an integrated manner and promote and develop closer working with the integrated local neighbourhood team in Stone.

7. To have a working knowledge of a variety of occupational therapy rehabilitation techniques, including both adaptive and restorative.

8. To maintain contemporaneous and accurate records on patient recording systems.

9. To maintain and develop own competency through a continuing professional development, maintaining a portfolio which reflects personal developments and meets HCPC standards.
  • To maintain and develop specialist knowledge of particular conditions and associated therapy through in-service training, self-directed learning, tutorials, peer review and external courses. To give feedback on training courses to colleagues.
  • To have an active role in multi-disciplinary team audit and audit specific area of practice as designated.
  • To have a sound understanding of clinical governance and risk management and apply to work situation e.g., lone working, moving and handling, incident reporting.
  • To work with the team in achieving team goals for service development.
  • To have clinical supervision on a monthly basis.
  • To use equipment to safely move patients such as hoists, stand aid when required.
  • To have occasional exposure to bodily fluids and unhygienic housing conditions
  • To use own car to occasionally transport patients and carry equipment.


Person Specification

QUALIFICATIONS & TRAINING

Essential

  • BSc in Occupational Therapy
  • Professional registration with HCPC


Desirable

  • Further clinical study relevant to clinical practice such as management of long-term conditions


EXPERIENCE

Essential

  • General experience across a wide range of care groups including neurology, orthopaedics, musculoskeletal and elderly care
  • Experience of working on own initiative.
  • Understanding of community-based working and systems.
  • Working in multidisciplinary team


Desirable

  • 2 years post qualification experience in the NHS.


SKILLS, KNOWLEDGE & ABILITIES

Essential

  • Knowledge of Occupational therapy with Parkinsons, MS, falls, cancers, frail elderly.
  • Daily living equipment assessment and fitting.
  • Confident, self-reliant
  • Ability to work alone and as team member.
  • Able to use reflective practice to identify training needs
  • Ability to compromise and adapt programmes to patient lifestyle and environment.
  • Able to present information in an understandable way for people with communication difficulties.
  • Ability to present information for patients with a range of ability to understand and act on instruction.
  • Ability to present information accurately, concisely & professionally.
  • Good IT skills and proficient in the use of Microsoft Word, Excel, Power Point and the internet.
  • Capable of cost and time efficient travel across a dispersed service area.
  • Able to carry equipment and patients in own car
  • Behaves in an open, ethical and professional manner.
