An exciting opportunity has arisen for a motivated enthusiastic Registered nurse to work as a part of the rheumatology services. To provide information, advice and support to patients with a rheumatological condition who are being monitored, until established on DMARD therapy.
The role requires running own clinics and also giving telephone advice on an ad hoc and rostered basis, recognising when onward referral/advice is needed. The nurse has a pivotal role in supporting multidisciplinary teams and coordinating the pathway for patients. This is a developmental role and there is an expectation that the post holder will continue their own professional development in line with personal and departmental requirements.
Main duties of the job
The post holder will be responsible for assessing, monitoring and reviewing patients with a rheumaological disease and who are receiving Disease Modifying Anti Rheumatoid drug (DMARD) therapy including oral biologic treatment.
To ensure that all patients receiving DMARD treatment are safely monitored in a timely manner, and according to local and national guidelines, and transferred to shared care as soon as is safely possible.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

To assess patients needs implement & evaluating programmes of care following local and national standards policies care pathways and protocols in particular NICE & BSR British Society of Rheumatology recommendationsTo work autonomously during clinic time assessing and monitoring patients including blood tests who are taking disease modifying drug therapy and reporting any abnormalities to a more senior member of the rheumatology team
Participate in patient education and promote self-management
Report and document any relevant patient needs concerns with other health care professionals who have ongoing responsibility communicating changes as they occur
Assist with the telephone advice line being aware of own limitations in managing patient problems and referring on as appropriate but using each intervention as an opportunity to develop own knowledge and skills
Demonstrate a commitment to evidence based practice and clinical excellence and play an active role in using this evidence to improve the patients experience
Take ownership of database so that information readily available regarding DMARD activity within the service. Ensure this information is also available to the wider rheumatology team
Adhere to NMC Code of Practice standards and record keeping at all times
Display an enthusiastic attitude to learning and a willingness to undertake additional training and development thus widening career progression & opportunities
