The Clinical Assessment Service (CAS) performs a pivotal role in admission avoidance. We are excited to be expanding our award-winning service to include the Clinical Single Point of Access (SPA). This role has a focus on avoiding unnecessary conveyance or admission to hospital by providing clinical assessment and support to the CAS service and the new single point of access within LCHS. Our aim is to support other health care professionals and patients to navigate through LCHS pathways and beyond, to improve their experience and to achieve the right care, first time.

We are looking for Senior Urgent Care Practioners to work within the Clinical Assessment Service and provide support and leadership across the divisions' emergency care division admission avoidance pathways. You will be based in Beech House in Lincoln at the heart ofthe Integrated Urgent and Emergency Division, but will be expected to attend other bases to provide support within these pathways when required. The Clinical Assessment Service/SPA provides telephone and virtual clinical assessments and navigation to patients and health care professionals who have been assessed by 111 or other services in Lincolnshire.

The Single point of access is being implemented in line with the Urgent and Emergency Care recovery plan to provide integrated care coordination to ensure that patients are directed to the right care setting, with the right clinician or team at the right time.

Main duties of the job

Successful candidates will need to demonstrate that they are able to meet the essential criteria in the person specification andcan provide evidence of experience in the treatment of minor illness and minor injury. Candidates will be willing to undertake a non-medical prescribing qualification.

We are looking for people who can demonstrate and role model the right values and behaviours and have excellent leadership and communications skills. You will be expected to provide leadership and clinical supervision to colleagues alongside other governance activities as detailed in the job description.

Telephone/Video consultations/face to face, offering advice and support to a wide variety of urgent care presentations over a 24-hour period.

Candidates will also hold a non-medical prescribing qualification.

Telephone/Video consultation/face to face, offering advice and support to a wide variety of urgent care presentations over a 24-hour period.

The role will be based at Beech House in Lincoln. The service runs 24 hours a day 365 days a year. There is a requirement to work a variety of shifts to cover the 24 hour period and this includes weekend work. Full time workers will be expected to work 2.5 weekends per month as the busiest times for the service are out of hours.

Person Specification

Qualifications

Essential

  • Registered Health Care Professional holding current registration with appropriate regulatory body.
  • Evidence of working to PGDs with advanced knowledge (AHPs)
  • Safeguarding-level 3 or willingness to undertake
  • Minor Injuries level 6/7 or very relevant experience
  • Minor Illness level 6/7 or very relevant experience
  • Non-Medical Prescribing qualification or willingness to undertake


Previous Experience

Essential

  • Evidence of recent and on-going personal development including obvious leadership development
  • 2 years post registration experience in relevant settings


Desirable

  • Evidence of governance activities
  • Evidence of previous leadership experience


Skills

Essential

  • Evidence of multi-agency & professional working
  • Basic IT / keyboard skills


Desirable

  • Use of System One electronic records
