We are looking for a motivated, dedicated and innovative individual who is ready to settle in the area of Ageing and Complex Medicine to join us to cover our acute and rehab medical wards, Haematology and Renal wards. Someone who thrives on getting the job done, making decisions and facilitating patients return to optimal function. You should have a passion for frailty and discharge planning, working closely with the MDT and especially Occupational therapy colleagues to assess, rehab and facilitate onwards flow for patients. You would see a variety of patients and be supported by band 7's and rotational band 6's to improve clinical outcomes and care provision for this group of patients. In return we will look to support your own development and leadership skills, you will supervise band 5's and students, support rotational staff and contribute to the wider team goals.

Main duties of the job

To work within the In-Patient Ageing and Complex medical therapy team across multiple ward areas.

To undertake advanced assessment and treatment of acute and rehab patients with multiple pathologies, who may have complex and/or chronic presentations.

To determine clinical diagnosis and treatment indicated and maintain records as an autonomous practitioner.

To lead assessment, liaison, and decision-making processes for patients.

To support the team lead and service manager in the planning, co-ordination, and evaluation of the service on a day-to-day basis, and to cover for any other area with this specific caseload as necessary.

Be able to deputise for the Band 7 in their absence and attend meetings as required.

To be able to work across 7 days and contributing to weekend working and contribution to the respiratory on-call rota.

To supervise and teach more junior members of the team and students on placement.

Have an understanding of respiratory and neurological assessment and treatment skills.

Qualifications / registration

Essential

  • Degree in Physiotherapy
  • HCPC registered in Physiotherapy


Desirable

  • Member of the CSP


Experience

Essential

  • Significant experience of working at Band 5 or Band 6 level or above within acute and community settings completing core rotations (Resp, Neuro, MSK, Community)
  • Undertaken postgraduate training relevant to ACM
  • Experience of working in complex MDT settings
  • Evidence of effective communication skills with patients with altered cognition and barriers to communication
  • Evidence of autonomous decision making in an complex medical setting
  • Evidence of assessing and treating a wide variety of patients with multiple pathologies in a in-patient clinical setting
  • Evidence of having worked in an in patient environment with Older and frail patients
  • Evidence of understanding of dementia and frailty
  • Evidence of supporting, developing and supervising qualified and undergraduate staff
  • Evidence of supporting equality, diversity and inclusivity for both colleagues and service users in a clinical environment


Desirable

  • Evidence based manual handling skills including using equipment to facilitate safe moving and handling

Essential

  • Evidence of advanced assessment and treatment of neurological /neurosurgical related conditions within a clinical setting
  • Strong evidence of supervision / competent clinical educator in a clinical setting

  • Evidence of quality improvement work or Service development within a clinical setting
