We are pleased to offer a vacancy for a Band 5 Occupational Therapist within the Bolton Memory Assessment team.
We are looking for someone who has an interest in memory assessment who will support the service in managing a small caseload of service users during the assessment and diagnostic process.
The role will include undertaking functional assessments in the home environment, assessments to aid the diagnosis of dementia and plan interventions around the individuals needs, using client centred and evidence based practice.
The OT will assist with the direction, supervision and support of the community Support, Time and Recovery Workers and the supervision of Occupational Therapy Students as required.
You will provide support and advice to carers, when and where appropriate and make referrals to appropriate services.
You will contribute within the wider MDT in the development of clear care pathways regarding support, care and treatment of the service user.
In return, you will receive line management supervision every 6-8 weeks from the senior OT , you will be supported with learning and development which includes team education sessions and CPD opportunities.

Main duties of the job
To participate in the assessment, monitoring and review of patients with possible cognitive impairment or dementia.
To manage a caseload of patients and undertake OT assessments and interventions and to contribute to team assessment processes, and making recommendations for future care provision as appropriate.
To select and apply individual and/or group treatment interventions to address patients needs in areas of adjustment to diagnosis, self-maintenance, productivity, leisure and quality of life.
Interventions will be undertaken within the persons current care environment e.g. day care, respite care, or patients current home in the community environment, as appropriate to individual patients needs.
Where appropriate, to collaborate with patients when identifying their needs, planning occupational therapy treatments and setting goals as part of their overall care plan.
To demonstrate a broad level of understanding of the effects of ill health and apply this when making recommendations to address the patients physical, psychological and social needs, including introduction of care packages.
To monitor and evaluate treatment in order to measure progress and assure effectiveness of the intervention.
To assess for, provide, install and demonstrate small aids and equipment and make recommendations for larger, more complex pieces of equipment and adaptations to Social Services Occupational Therapy Team.

Job Summary/Purpose
- Under the line management and clinical supervision from within the Team, the post holder will:
- Participate in the assessment, monitoring and review of patients with possible cognitive impairment or dementia, as defined in the NICE guidance.
- Manage a defined caseload of adults of any age presenting with organic impairment with complex mental and physical health needs, using evidence based/person centred principles to assess, plan, implement and evaluate interventions, for service users living in the community and in line with the relevant dementia clinical pathway as outlined within the Teams operational procedure. To assist with the direction, supervision and support of support staff within the team and the training of Occupational Therapy Students as required.
- To abide by the College of Occupational Therapy Code of Ethics and Professional Conduct and the College of Occupational Therapy Professional Standards for OT Practice.
