The Continuing Health Care (CHC) team are in search of a qualified social worker to work with our team to support the completion of DOH Decision Support Tool (DST) in determining eligibility criteria for continuing health care funding. This will involve constant interaction with diverse groups of people and individuals, including the wider multidisciplinary team.

A successful Social Worker candidate will have various prerequisite skills and qualifications that typically include:

Social Worker to work as part of the team with individuals, families and the MDT in completing a comprehensive assessment of need. The social worker will work within the Continuing Health Care team, including visiting patients and possibly nursing homes.

Strong knowledge of the continuing health care framework and how it applies to individual health and social care needs

Empathetic nature to adequately understand clients predicaments and provide the necessary support

Excellent organisational skills to successfully priorities the many different aspects of the job

Patience to successfully deal with complex cases and clients who may not progress as expected

Superior emotional intelligence and maturity to cope with the emotional demands of the job

Effective analysis, assessment and planning skills

Main duties of the job

The main purpose of the social worker role is to provide aprofessional social work service to adults, their families andcarers in line within the requirements of the Care Act 2014.The role holder will assist individuals, carers and families bycompleting strength based assessments and support planswhich support the identification of individual, personalnetwork, community strengths and universal services.

Applying robust and evidence based eligibility decisionmaking and promoting best value. Intervention is tobe holistic, looking at the whole person and all relevantprofessionals around them as to work in an integrated way.

Social workers will also undertake safeguarding enquiries inline with legislation, statutory guidance and good practiceapproaches with a focus on making safeguarding personalto ensure that the adult is at the centre of the process. Thiswill include identifying, responding to, managing andevaluating risk as appropriate considering and applying theMental Capacity Act 2005 as appropriate.

Person Specification Knowledge Essential
  • Working knowledge of national policies and the relevant legislation, including Care Act 2014, Mental Health Acts, Mental Capacity Act, equalities and diversity and theories underpinning the provision of services to the relevant client group.
  • Working knowledge of financial and legal frameworks, data protection, GDPR.
  • Working knowledge of the London Multi Agency Safeguarding policy and procedures.
Qualifications Essential
  • CQSW, DipSW or other GSCC/HCPC recognised social work qualification, and current Social Work England registration
  • Demonstrable ability to work in a person centred outcome focused way
  • Demonstrable experience of ensuring the full and active participation of users and carers in their own care arrangements.
  • Demonstrable experience of working in conjunction with other health and social care agencies and professionals
  • Demonstrable experience of using community resources, family and natural networks of support to create sustainable packages of care for individuals and help prevent, reduce or delay the need for statutory services.
  • Able to demonstrate an understanding of the risk indicators of different forms of abuse and neglect and their impact on individuals, their families or their support networks.
Additional Requirements Essential
  • Willing to undertake evening and weekend work when required, subject to the requirements of the service
  • A DBS enhanced disclosure that is satisfactory to us will be a condition of your employment
  • Willing to travel and work flexibly across various sites and locations in response to service demands
  • This role may require visiting care homes and is in scope of the Health and Social Care Act 2008 (Regulated Activities) (Amendment) (Coronavirus) Regulations 2021.
