We are looking for a Specialist Doctor to join our East Kent HCP Frailty/Home Treatment Service.
Our diverse, talented and caring team include frailty consultants, a consultant advanced clinical practitioner (ACP), GPs, consultants-in-training and trainee ACPs from a broad professional background.Together we support a wide range of activities including:
Main duties of the job
The broad responsibilities of the post will include:
Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence Leadership and consultancy providing expert professional advice to patients, carers and colleagues Development and provision of education and training to other staff and trainees Service development, evaluation, research and audit. Shared decision-making with families, trainee staff and external organisations MDT collaboration within and across organisational boundaries Supporting the development of their team and demonstrating leadership abilities Contributing to organisational learning Critically appraising current research and sharing best practice

- Full GMC registration

- A post graduate qualification e.g. MRCP, Diploma or Masters qualification relevant to the role.
Experience

- 12 years post qualification experience, continuous or in aggregate that is acceptable for full, limited or temporary (but not provisional) GMC registration including 3 years of community frailty work. There may be flexibility with this depending on skills and experience. Extensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience) Independent prescribing experience Experience of advanced specialist interventions in the assessment and management of older patients Experience of teaching and education provision Extensive experience in report writing Experience of planning, implementation and delivery of educational programmes relating to long term conditions Experience of working across organisational boundaries with partners (commissioning, social care and education providers) Experience with evidence of audit, research and innovation applied in practice Experience of strategy development and implementation
