The post of Lead Practitioner will be deployed across the Criminal Justice Liaison & Diversion service. We are looking to recruit individuals with expertise in the assessment of people across all vulnerabilities, including Substance misuse, Learning Disability and Children and young people with core skills including assessment and signposting.

The service works with people within the criminal justice system where potential mental health and or learning disability need is suspected and further assessment is indicated to inform disposal of the case and to identify robust care pathways.

A commitment to close partnership working with both statutory and voluntary sector agencies and a commitment to working with a complex and diverse range of individuals with varying needs is vital to achieve the service's objectives.

Main duties of the job

The Award Winning Liaison & Diversion Team is looking for dynamic and innovative Lead Nurses with expertise in the assessment of people across all vulnerabilities, including Substance misuse, Learning Disability and Children and young people with core skills, including assessment and Signposting.

The post holder will have relevant qualifications and experience. The team are based within the police custody suites and magistrates courts across Derbyshire providing support, assessment and signposting to individuals within the criminal justice system from suspicion of offence to disposal. L&D is an all age all vulnerability service from the age of 10 upwards presenting with a wide range of needs.

You will be working alongside an established team of professionals all with expert knowledge and experience of meeting the needs of individuals throughout the criminal justice pathway across the 'all vulnerabilities 'model.

From 11 November 2021, everyone working in care homes has to be fully vaccinated against Covid-19 by law to protect residents. As a provider of services to CQC registered care homes, we shall only employ you if you are fully vaccinated in roles that require you to work in a DHcFT-led core unit or care home setting (unless you are medically exempt). Mandatory vaccination will be part of your terms and conditions of employment (unless an exemption applies)

To attend police custody suites and magistrates courts and provide mental health assessments, advice and identify robust care pathways, for individuals where a potential mental health and or learning disability need has been identified.

To provide the police and court with written information pertaining to the individual in a timely fashion to inform disposal

To work closely and build links with all referring agencies within the custody suite, court and probation acting as a single point of contact for health

To address the court in person when necessary in order to advise the court in the disposal, and care pathway of a particular case.

To refer to the appropriate team within DHCFT community mental health teams

To make recommendation to the Crown Prosecution Service or court, whether the criminal justice system remains involved ,or diversion is indicated

To build a good working knowledge of the mental health act in relation to the police, courts and the criminal justice system

Where diversion is indicated, to facilitate mental health act assessments at the police station or court in conjunction with the DHCFT assessment teams

To refer to the prison in reach teams where custody is indicated ensuring appropriate information is received along the care pathway

To provide specialist advice and service to probation and the custody suites where indicated.

To provide specialist advice / assessment to community probation teams.

To provide mental health information to probation in the formulation of their pre-sentence reports including the use of Mental Health Treatment Requirement where a potential mental health need has been identified

To be responsible for adhering to all DHCFT policies and procedures.

To be able to communicate clearly with a diverse group of service users.

To work autonomously, with the ability to organise and manage own time according to delegated workload.

Take accountability for own professional practice.

To possess a good working knowledge of the Integrated Care Programme approach (ICPA) including the ability to formulate a risk assessment and risk management plan.

To provide formal and informal clinical supervision to junior staff, students, mental health workers where appropriate/in agreement with the team manager

To facilitate the efficient operation of the team, including the provision of cover for the role of service manager in their absence as required

The role will require the post holder to travel across Derbyshire to cover the custody suites in Derby, Chesterfield and Buxton, driving is essential.
