We are looking for an enthusiastic band 5 nurse to join our South Molton/Chulmleigh Community Nursing team. You will visit patients in their own homes, giving good holistic care and working within the Health and Social Care teams to give good evidence based collaborative care to our patients.

We are looking for enthusiastic and committed Registered Nurses with excellent communication skills to work with a Community Nursing team based within the South Molton/Chulmleigh area. The successful candidate will ensure patients are cared for safely in their own home or other appropriate community settings providing evidenced based nursing care, ensuring patients are supported and enabled in maintaining independence. If you have experience or an active interest in Community Nursing, preventing hospital admissions and managing people in the end stages of their life or with chronic diseases then come and join our team.
Community Nursing encourages you to think on your feet and helps you develop and increase confidence in your ability as a Registrant. We encourage our staff to develop and to attend training courses to gain the extra knowledge and skills that will make you the best Nurse you can be. We are a supportive team who thrive to give 100% for our patients. We support and encourage each other and that is what makes this team so great to work for.
You will be work as part of a team but would be required to be an automatous Practitioner.
We offer an excellent preceptorship, and all Nurses will have a robust induction whether you have previous experience or not.
Working Pattern: 37.5 hours per week, 09:00 - 17:00 and working on a rota for weekends which currently is around one weekend a month

Pay:The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings (Pay scales for 2023/24 | NHS Employers).
