Physiotherapist - Community Stroke Rehabilitation

We are looking for a Physiotherapist with a keen interest in stroke rehabilitation to join our Community Stroke Team in Guy's & St. Thomas' NHS Foundation Trust.

Our service aims to provide high quality and goal-orientated specialist rehabilitation and disability management for individuals with stroke living in Lambeth and Southwark. We are a multi-disciplinary service consisting of Occupational Therapists, Speech and Language Therapists, Physiotherapists, Rehab Support Workers, Clinical Psychologist, Stroke Nurse, Stroke Care Advisor and Stroke Reviewer.

We are part of a larger Community Stroke and Neuro Rehab Service clinically led by an AHP Consultant and a Consultant in Neurorehabilitation. We have an integrated specialist Spasticity Management Service, 2b Inpatient Neurorehab, Community Neurorehab Team and Neuronavigation. We participate in research studies such as the TRICEPS (TRanscutaneous lImb reCovEry Post-Stroke) trial.

We have several community working bases across Lambeth and Southwark with the primary clinical base for this post being in Brixton. The position is static. We support agile working as each staff member will be provided with their own mobile devices.

Main duties of the job

You will be a Physiotherapist with postgraduate experience working with adults with stroke and acquired neurological impairments (physical, sensory, cognitive, and communication). The position will suit individuals keen to enhance their specialism in rehabilitation and disability management post stroke. Ideally, you will have some community neurological rehabilitation experience. You will predominantly see patients in their own homes but will also be expected to utilise other community venues such as non-NHS gyms and our own clinic and gym space in Brixton. You will also be expected to contribute service improvement initiatives through audit and quality improvement projects. For example, the Stroke Physiotherapists are currently working collaboratively on pathways to support the diagnosis and management of post stroke shoulder pain.

The post holder will:

• Work as an autonomous practitioner within the standards of professional practice laid down by the Chartered Society of Physiotherapists (CSP) and Health and Care Professions Council (HCPC). Be professionally and legally accountable for all aspects of own work.
• Maintain close liaison with other senior colleagues in the team and ensure effective communication with all team members for day to day smooth running of the service.
• Keep abreast of current issues with the profession, taking responsibility for personal growth and development such as reading relevant journal and research papers or participating in appropriate work journal clubs and related research / evaluation projects.
• Maintain and record ongoing continuing professional development, using a professional portfolio.
• Attend professional body / clinical interest group meetings as appropriate.
• Participate in clinical training for Physiotherapy students.
• Attend training courses, participate in experiential learning as identified in PDP and impart information to colleagues.
• To provide a comprehensive assessment, treatment and management service to a caseload of adult patients with a wide range of acquired neurological conditions following local operational policies and agreed standards and guidelines.
• To be responsible for prioritising and managing own workload. To achieve expected individual activity target (face to face contacts) of at least three patients a day.
• To work collaboratively with clients and carers, members of the multidisciplinary team and external agencies to ensure a co-ordinated approach to service delivery. To discuss, advise and plan appropriate patient goals with clients, carers and the multidisciplinary team
• To attend multidisciplinary team meetings, case conferences, family meetings and ward meetings as necessary and other professional meetings as requested by the line manager
• To respect clients' dignity, privacy, culture and wishes during treatment, with a commitment to equal access to rehabilitation.
• To maintain and develop close links with colleagues in the Acute sector, Local Authority Services, Voluntary Organisations and other members of Primary Health Care Services.
• To supervise rehabilitation assistants where appropriate.
• To undertake monthly supervision with highly specialist physiotherapist.
• To attend all appropriate team meetings.
• To participate in the Trust staff appraisal scheme.
• To be responsible for safekeeping and cleanliness and maintenance of equipment ensuring standards of infection control are met and reporting all equipment defects, accidents and complaints to the Clinical Manager.
• To participate in ongoing review of service delivery and any ensuing service developments.
• Keep accurate and contemporaneous records of assessment and treatment in accordance with standards set and input data onto electronic record system.
• Provide accurate and timely statistical data.
• Contribute to the planning, organisation and delivery of training for staff in the Neuro Rehab Service and from other disciplines and organisations through in-service training, supervision, joint working, practical demonstrations, etc.
• Contribute to student education and development.
• Assist with service development and service promotion.
• Undertake all mandatory and statutory training in line with the Trust policy.

Experience

• Significant broad based post registration experience at B5 level or above
• Experience of a variety of clinical specialties including neurological and stroke rehabilitation
• Experience of working within a multi-disciplinary team (MDT)

Desirable

• Experience of working within the community
• Participation in service development or project work
• Experience of working in the NHS

Skills

Essential

• Knowledge, understanding and application of Physiotherapy process relating to client-centred practice
• Demonstrable Skills in assessing patients with complex conditions, identifying the problems and selecting and implementing the relevant treatment/management programme.

Knowledge/Qualifications

• Graduate Diploma or Degree in Physiotherapy
• State registration with HCPC
