Employed by East Lancashire Hospital NHS Trust, you will work alongside an existing and busy Paediatric diabetes team. This is an exciting opportunity for an experienced Paediatric diabetes nurse specialist to work along side this friendly dynamic team. Working with the team you will manage and continue to operationally develop the Children's Diabetes Service in order to facilitate the early discharge and prevent avoidable re-admission to hospital for children with diabetes, through the provision of specialist support. You will hold a full case load and provide highly specialized clinical advice and support, education and training to healthcare professionals and others.

Main duties of the job

Support the Education Lead nurse in planning, preparing and delivering

Diabetes Education to children and young people and their families.

Build strong links with nurseries/schools/colleges and support teaching

staff.in managing the specific health needs of children/young people with Diabetes.

Support families both at home and in the community in managing their

child's diabetes regime.

Provide targeted support for children and young people with a sub optimum

Hba1c.

Jointly lead the team alongside the other band 7's and support other members of

the team. Toidentify families with high risk indicators and follow trust

policies regarding safeguarding children and young people.

Ensure up to date documentation on electronic system CERNER. Undertake

training.

Services are delivered from both Royal Blackburn Hospital and Burnley General Hospital as well as from a range of community clinics, Child Development Centres and Children's centres across East Lancashire and Blackburn with Darwen.

Job responsibilities

Overall continuing responsibility for the clinical assessment and autonomous decision-making with regard to individual care needs, within hospital and community settings.

Co-ordinate and delegate, as appropriate, nursing activities to other

members of the Childrens Diabetes Nursing team.

Monitor the management and delivery of care to children and young people and their families by members of the Childrens Diabetes Nursing Team.

Act in partnership with the child and family to continuously assess a childs health, emotional and social needs. Plan, implement and evaluate care in accordance with the Directorates Philosophy of Care. Manage and resolve differences of opinion between families, healthcare professionals and other careers regarding any issues, acting as the childs advocate at all times.

To provide highly specialist support and education for children/young people, families and other careers, at diagnosis and thereafter, to enable them to become confident in their abilities to meet the childs health needs at home and in other community settings, e.g. in college/school, during recreational activities such as sports etc. and respite care.

Provide a specialized service for continuous insulin pump patients and their families within ELHT.

Provide a specialized service for continuous glucose monitoring (rt CGM) for patients and their families within ELHT.

Provide a specialized service for flash glucose monitoring (FGM) for patients and their families within ELHT.

Provide highly specialist nursing and emotional support for children, families and other careers during distressing times of a childs life e.g. at diagnosis or when their condition deteriorates for whatever reason, working with Clinical Psychology staff when indicated.

Provide complex technical and theoretical information in appropriate manner to facilitate the understanding of individual children/young person and their families and other carers, to enable that understanding to be put into practice.

Motivate and empower children and their families and other careers to manage this condition.

Refer to/work with other specialist services/agencies as indicated or as necessary e.g. Social Services, Interpreter. Attend inter-agency meetings and review meetings when required. Lead on inter agency meetings if indicated.

To advise families on titration of insulin doses according to local protocols, working closely with the Diabetes Consultant team. To have or a willingness to work towards non-medical prescribing qualification.

Order relevant laboratory investigations and diagnostic procedures.

Lead on the nurse led clinics applying continuous glucose monitoring, interpreting the results, and communicating with other health professionals as indicated.

Oversee the organization and running of diabetes clinics. Contribute to diabetes clinics, and lead nurse/dietitian clinics.
  • Facilitate and support the provision of transition services for young people between paediatric and adult service providers.

Act as a prime clinical resource and support person for other members of the diabetes team, and other professionals in the paediatric in-patient hospital team and primary care team members.
