We are looking for a Consultant Advanced Clinical Practitioner to join our West Kent Home Treatment Service.

The Consultant Practitioner for Home Treatment Service leads the Home Treatment Service alongside the SAS Doctors West Kent.

We are a community team made up of a variety of staff including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners and Health Care Assistants. The Home Treatment Services have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust and Acute Trust Teams.

Main duties of the job

You will be a clinical expert in Frailty and will fulfil the 4 domains ofAdvanced Clinical Practice:

1. Advanced and expert level clinical practice within the field of frailty ensuring clinicalexcellence2. Leadership and consultancy providing expert professional advice to patients,carers and colleagues3. Development and provision of education and training to other staff and trainees4. Service development, evaluation, research and audit.

As a member of the MDT, you will also be involved with, and where appropriate collaborate in national and international research contributing to relevant networks and forums, with the aim of supporting the development of a centre of excellence in care and treatment for older people.

You will maintain and further develop a local and national profile as a Consultant ACP for Frailty, by being a creative and innovative leader in further developing services for this patient group

Additional Working Pattern: 0900 to 2200 3 shifts a week including weekends

Person Specification

QUALIFICATIONS

Essential

  • o First level Registration with NMC, HPCP or GPHC o Minimum of 5 years post registration experience, Experience at band 8a or above o Educated to Master's level or higher degree o Teaching qualification o Research qualification o Leadership o Non- medical Prescribing


Desirable

  • o Psychology or counselling experience and training/ education


EXPERIENCE

Essential

  • o Extensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience) o Advanced skills in history taking and physical assessment of frail people o Independent prescribing experience o Experience of advanced specialist interventions in the assessment and management of older patients o Experience of teaching and education provision o Extensive experience in report writing o Experience of planning, implementation and delivery of educational programmes relating to long term conditions o Experience of working across organisational boundaries with partners (commissioning, social care and education providers) o Advanced IT skills o Experience with evidence of audit, research and innovation applied in practice o Experience of strategy development and implementation


Desirable

  • o Extensive experience of presenting report/ education/ information to public forums o IR(ME)R training or equivalent


KNOWLEDGE

Essential

  • o Advanced understanding of long-term conditions and application to older people o Advanced knowledge of clinical governance o Advanced knowledge of change management o Advanced knowledge of managing complex patient information and situations
