Mental Health Practitioner:
In Hampshire and Isle of Wight (HIOW) we have launched our plan to transform experiences in adult mental health through a programme called ‘No Wrong Door’. This is led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness.
Adults with a mental illness have told us first contact is the most important point in successfully starting their recovery. When people with a mental illness are at their most anxious, they face too many choices with little support. To tackle this, we are going to:
1. Ensure that wherever and however a person accesses care, they are met with compassion, understanding, and supported with both their physical and mental wellbeing.
2. Build teams around communities that are close to people’s homes, making sure people’s recovery is close to their personal support networks.
3. Provide people who are working across these physical and mental health services with the right support and training, making sure we have creative career options across the area.
Southern Health NHS Foundation Trust (SHFT) will be working with Avon Valley Primary Care Network (known as PCN), which looks after the needs of the local population in the New Forest and works in collaborative partnership with other organisations including local authorities, community service providers and voluntary and community groups to deliver this new and innovative approach to mental health care.
You will be part of an ambitious but compassionate and friendly team of professionals in various additional roles that have been created to enhance our mental health care. You will work as an autonomous practitioner within this team, developing close working relationships with General Practitioners and be a valued member of the primary care team. The role offers exciting opportunities for direct clinical work, service development and multi-disciplinary team working. You will work with Primary Care stakeholders in South West area to support and deliver therapeutic interventions to service users in primary care who do not meet the criteria for secondary care services.
Your role will be to support the planning and coordination of mental health in primary care for people presenting to the GP practices with a range of mild to moderate mental health needs and deliver therapeutic interventions. Work with the secondary care mental health practitioner and help lead the PCN team of Social Prescribers and Care Coordinators in providing a support service for patients in the Avon Valley
