An exciting opportunity has arisen for a Band 5 RMN Nurse to join Evesham Neighbourhood Community Mental Health Teams (NMHT).
Contributing to the delivery of our services, facilitating group interventions and working with individuals on improving the functioning in their lives, as a key member of our MDT across the Primary Care Networks ensuring our patients receive expert care focused on recovery.
The post is for 37.5 hours a week Monday to Friday

Main duties of the job

Job description
Job responsibilities
Under the direction of a nominated supervising professional:
- Provide specific Psychiatric Nursing interventions to allocated patients/clients and their relatives who are referred to the service.
- Work in conjunction with all primary care services and partnership agencies to foster close multidisciplinary and multi-agency working.
- Participate in regular supervision with nominated supervisor and evaluate caseload.
- Assess care needs, develop, implement and evaluate programs of care utilising the Recovery model.
Clinical
- Function as an active member of the Community Assessment Recovery Service.
- Ensure high standards patient care, ensuring patients individuality and dignity is maintained at all times, while promoting independence.
- Maintain up to date, accurate records within Trust and NMC guidelines and keep these confidential and in a safe place.
- Responsible for ensuring risk assessments are carried out in conjunction with other agencies.
- Ensure that the philosophy of Person-Centred Care is upheld at all times.
- Fully implement the agreed care plan to include commissioning where required, with the support of more senior health and social care staff and ensure that it is regularly reviewed and updated.
- Ensure that the requirements of the Mental Health Act 1983 are met and that patients rights under the Act are upheld.
- Participate in all aspects of discharge and aftercare including co-operation with Section 117 meetings and the Care Programme Approach and Care Co-ordination.
- Ensure effective liaison with all agencies to ensure seamless service provision.
- Ensure that information regarding patients needs and progress are communicated promptly and accurately to other team members involved in their care, having due regard for confidentiality.
- Facilitate open and harmonious communications amongst staff within the team and with other departments with the aim of promoting multidisciplinary approaches to care.
- Participate with direct support of a senior colleague in undertaking the role of Professional Cover.
- Ensure that each patient in his/her care receives a comprehensive multidisciplinary CPA assessment in line with the CPA process and is the subject of a regular updated care plan which is responsive to changing needs. Where practicable, this care plan must be formulated in conjunction with the patient.
