The role will be working in the community with stroke patients. You will have opportunities to work closely with other multidisciplinary teams in the community, primary care, Acute Neuro Rehab Teams, the Community Neuro rehab Team working with complex neurological problems and progressive neurological conditions and other external agencies. There is an in-service training programme in place. The post is within a multidisciplinary team and you will work alongside our Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists and Rehab assistants.

This is a countywide post and can therefore be based from any of the ESD bases, dependent on closeness to their home. The successful candidate will work in an agile manner with a combination of face to face and virtual consultation to manage their caseload of patients, having access to a smart phone and laptop to support this.

Main duties of the job

We are seeking an enthusiastic and dynamic Specialist Neuro Rehab Physiotherapist to join the Team. This role is to support the delivery of community neuro rehab to stroke patients in both inpatient rehab beds and in their homes. Close multidisciplinary work is a key way of working, providing a supportive environment for all staff. You will support patients with physical, cognitive and psychosocial challenges to achieve their potential and will be instigating and delivering better health, care and improved efficiency for our patients.

Staff personal and professional development is highly important to us and staff have the opportunity to receive training through in-service programmes as well as being supported to undertake a blended competency package to help you develop within your profession and role.

The ideal candidate will be flexible and adaptable to support service delivery.

You will be expected to travel independently round the county, please confirm in your application that you meet this criteria. This can not be using public transport as this is not a reliable form of transport and can not meet service needs.

  • To assess, plan and develop specialist physiotherapy programmes for patients under care of the Early Supported Discharge Service for Stroke (ESD).
  • To work as an autonomous specialist practitioner within the multi-professional ESD as part of the Countywide Neuro Rehab Service.
  • To be responsible for own caseload and undertake assessment of stroke patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills
  • To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional and psychological needs. Provide appropriate equipment and rehabilitation programmes and make referrals on to other professionals and agencies as appropriate in order to meet the identified needs.
  • To be involved in coordinating and providing 6 month reviews for stroke survivors.
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • To allocate work to junior and non-registered staff
  • To provide specialist neuro physio advice to professionals from other disciplines.
  • To provide support and education to peers, new staff non-registered staff and students.
  • To supervise junior staff and students, overseeing patient intervention and ensure that Assistant Practitioners, Rehab Assistants and Integrated Care Workers deliver high quality patient care.
  • To be actively involved in training, teaching and supporting band 6 and 5 therapists, Assistant Practitioners, Rehab Assistants and Specialist Integrated Care Workers to achieve competencies.
  • To be accountable for a delegated caseload held by non-registered staff in the team.
  • To build up and maintain key relationships with referring services and Community Neuro Rehab Services to facilitate appropriate referrals, and an understanding of the roles within the Early Stroke Discharge team
  • To take an active role in the stroke rehabilitation duty process to prioritise and ensure a timely response for assessments.
  • To provide a high standard of care.
  • To work in various community settings including community inpatient beds and patient homes; this involves lone working
  • To lead on the development and operational implementation of NICE guidelines, Clinical Pathways and Training/Education.


Person Specification Education / Qualifications Essential
  • Current HCPC registration
  • Fieldwork/Practice Educator qualification
  • Degree or equivalent, e.g. BSc, MSc or Diploma entry level qualification into the Physiotherapy profession.
  • Professional knowledge in Neuro Rehabilitation including Stroke rehabilitation and training to post graduate qualification or equivalent experience
  • Theoretical study and experience in Neuro Rehabilitation including Stroke to post graduate qualification or equivalent experience
Desirable
  • Member of CSP
  • Member of ACPIN
  • Management/Leadership qualification and experience
  • Clinical Assessment Skills for Health Professionals
Experience Essential
  • Extensive relevant post-graduate neuro rehab and stroke clinical experience in a variety of settings including community and working across service boundaries.
  • Specialist knowledge of a broad range of physiotherapy disciplines
  • Advanced understanding of stroke and neurological conditions with experience of managing patients with complex neurological problems including cognitive deficits
  • Experience of being involved in research study
  • Experience of implementing change
  • Experienced Clinical Supervisor
  • Previous NHS experience or equivalent at this level

  • Able to carry out moderate to intense physical effort throughout the working day and carry out concurrent activities including working in confined environments to deliver therapy
