Swansea Community Integrated Services is a vibrant and dynamic integrated team that aims to facilitate timely discharges from hospital and prevent avoidable admissions to secondary care. This post sits at the heart of Welsh Governments desire for 'A Healthier Wales' and supports Home First initiatives.

Main duties of the job

The clinical lead role is varied and challenging and is supported by an excellent clinical team of other Therapy Leads, Therapists, Assistant Practitioners, , Administrators, and a clinical/professional support structure. Professional and operational leadership is provided by the Interim West Hub Manager. You will work with peers from other localities and clinical teams such as District Nursing, Acute Clinical Teams, Home Care, Social Work, and hospital therapy teams.

The successful candidate would be joining an experienced team of friendly and forward thinking clinicians that can providesupervision, regular appraisal and clinical supervision opportunities, regular physiotherapy and multi-disciplinary team meetings, active support for research, audit and evidence-based practice and dedicated time and funding for appropriate CPD.

The post holder will participate in a 7 day working roster.

The post holder must have a Physiotherapy degree qualification, be a member of CSP and be registered and licensed to practice with HCPC.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The ability to travel in a timely manner across Swansea Bay is essential for this community post.

  • Relevant current professional registration NMC / HPC
  • Relevant Degree/Dip for Profession. Plus Masters Degree or equivalent experience.
