Integrated Community Reablement and Assessment Service (ICRAS) - exciting opportunities to join an evolving service as a Therapy Coordinator, with an Physiotherapy background.

Main duties of the job

The post holder will provide expertise and advice in the treatment of complex conditions and have continuing responsibility for the management of a defined caseload and a coordination role of daily activities. Responsibility for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.

Practice autonomously and demonstrate evidence based clinical decision making.Work within guidelines to identify and manage risk, reporting identified risks to the team leader. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.In addition the post holder will use their expertise to be a source of training and mentorship to those within the team and as necessary, more widely.

Principal Responsibilities:1. Responsible for the provision of appropriate, high quality care to a defined caseload.2. Responsible for workload planning and delegation of work to other team members, takinginto account competence levels.3. Provide care, advice and guidance in the treatment of others, when dealing with complexhighly specialised conditions.4. Undertake holistic assessments of need for the patient and family, which may be highlycomplex, using the appropriate model of care and evidence based practice.5. Work in partnership with other professionals to enable patients/clients to be maintained inan appropriate environment, thus avoiding hospital admission. Including stakeholders/LCCand external agencies.6. Provide advice to patients, clients and carers on a wide range of health issues to includehealth promotion, life style advice and health care.7. Ensure that the team provides a high quality clinical service to its clients by participation incaseload and clinical supervision.8. In conjunction with the team leader, develop new ways of working to provide a responsiveservice to patients with both planned and unplanned care needs.9. Develop partnerships and joint working within other agencies as appropriate.10. Maintain registration in line with professional bodies.11. Clinically lead, direct, mentor and support the team.12. Support the team leader in the development of caseload management across the localhealth economy.13. Acts as an advocate and champion for patients and/or clients.14. Act as a role model to team members and students so that patients/clients receive the most effective care possible.15. Contribute to the development of policy and services to reflect the needs of the localneighbourhood population.16. Participate in the development, implementation, and monitoring of the clinical teamcontract/objectives.17. Provide information to the team leader on staff and patient/client activity as requested, to enable the effective management of the team.18. Support the team leader in the implementation of practice development, improvement and service modernisation initiatives.19. Support the team leader by monitoring and recording sickness and absence, capability and other issues within the team, reporting them as soon as possible.20. In conjunction with other caseload holders, provide day-to-day supervision and support for the team.21. Support the team leader in providing induction to the local working environment, clinicalservices and policies for new team members and students.22. Undertake personal development plans with other members of the team, as delegated bythe team leader.23. Act as an authorised signatory for mileage and time cards for submission to the team leader.24. Communicate with the multi-disciplinary team to provide best outcomes for patients/clients;this may include organising case conferences related to highly complex cases.25. Participate in individual and group supervision.26. Monitor record keeping within the team and inform the team leader of any issues.27. Support the training and development of team members who need to improve on the level of patient care delivery. This includes record keeping and improving clinical competency.28. Responsible for the delivery of identified specialised training.29. Support the implementation of any initiatives to improve quality within the clinicala. governance framework.30. Participate in patient satisfaction reporting to improve patient care.31. Work with the team leader to ensure achievement of your own personal development plan.32. Maintain up to date knowledge and competence in line with professional and servicerequirements and demonstrate critical thinking, decision-making and reflective skills toensure own professional development.33. Challenge professional and organisational boundaries to ensure the role is focused onmeeting the needs of our patients.34. Support the Team Leader in creating an environment in which learning and practicedevelopment is fostered, evaluated and disseminated.35. Facilitate effective learning within the area of practice for all students and practitioners.36. Facilitate pre and post registration student placements within the team.37. Mentor and assess pre-registration students.38. Participation in clinical audits and research, as required.39. The post holder shall as necessary provide cover for and undertake duties of absentcolleagues.40. The post holder shall follow all the policies and procedures of the organisation.41. All employees have a personal contribution to reducing healthcare-associated infections (HCAIs). Staff must be compliant with all measures known to be effective in reducing HCAIs and be familiar with the Trusts Infection Control Policies. All staff are required to attend a mandatory training session on infection prevention and control and be updated according to their area of employment.56. Infection Prevention and Control is everyones responsibility. All staff, both clinical and non clinical, are required to adhere to the Trusts Infection Prevention and Control Policies and make every effort to maintain high standards of infection prevention and control at all times thereby reducing the burden of Healthcare Associated Infections including MRSA and Clostridium Difficile in accordance with the Hygiene Code - Code of Practice for the Prevention and Control of Healthcare Associated Infections (DH 2008)

57. All staff employed by the organisation have the following key responsibilities:-Staff must wash their hands or use alcohol gel on entry and exit from all clinical areas and/orbetween each patient contact.- Staff members must attend mandatory infection prevention and control training provided for them by the Trust.- Staff members who develop an infection (other than common colds and illness) that may betransmittable to patients have a duty to contact Occupational Health.58. Person Specification Qualifications Essential
  • Qualification of relevant degree to the post requirement.
  • Registration with relevant professional body. Teaching/mentorship qualification or equivalent or equivalent work based experience. Evidence of CPD/short courses
