Are you a dynamic, enthusiastic and experienced Band 6 clinician looking for a part time role? Then look no further! Come join our team in Basingstoke CMHT Meadows.
This role is to manage a caseload of patients through recovery to discharge, signposting back to the community.
Managing two clinics which are currently on a Tuesday and a Wednesday however this can be flexible.
Receiving referrals from our (multi-disciplinary team) MDT and Psychology.
Supporting patients during psychological therapies and after. Working with low-risk patients but monitoring and reviewing.
Working 9-5pm (Can be flexible hours). Regular supervision and opportunities off training and further education.
A chance to develop your skill set and share your knowledge and experience with the team.
Main duties of the job
Patients who do not need medication to be reviewed by a doctor -- dose is optimized unless the review is requested by NLC. Medics can set prescriptions plans and this will can be monitored in NLC.
*Patients who are stable but need ongoing monitoring i.e. for bipolar disorder.
*Patients who are recovering, no longer needing a Care Coordinator intensive support but not ready for transfer.
*Patients who are clinically ready with once a month or 12 weekly appointments and are on the transfer pathway back to primary care pathway.
*Patients who need monitoring whilst under the care of psychology or a support worker is delivering a care plan, or awaiting a psychology group or during a psychology group and the NLC will then transfer (to primary care) on completion of the therapy.
*Patients may be supported to complete my crisis plan this is part of recovery focused discharge planning. As well as using dialog + Care plans with the patient making it person centred care.
*Patients will be offered local signposting as part of the recovery process in finding local support groups and social groups as indicated.
Liaise with primary care colleagues PCN for step down care or transfer if appropriate.
