The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care within the framework of the Core Fidelity model.

The post holder will operate as part of an integrated Home Treatment Service but be expected to be flexible in terms of delivering care within a variety of environments.

The team will work to assess and provide treatment to service users with complex needs. Alternative options to admission will be considered where possible.

The post holder will demonstrate an attitude which respects and values service users' choice and works along side them and their carers to ensure they are at the centre of care planning.

The post holder will be expected to participate in a 24hr roster system 7 Days a week, on a rotational basis as and when the service demands.

Main duties of the job

The CRHT will offer two specialist pathways Crisis and Home Treatment as part of a multi-disciplinary approach for the Wigan Borough. The service will be 24 hours and the main base will be at Atherleigh Park.

The role will include:
  • Management of a case load within the Crisis Resolution /Home Treatment Team
  • Generic and specialist assessments, including risk and provide therapeutic interventions/ home treatments within the most appropriate setting in accordance with the service users and/or carers' needs.
  • Gatekeeping for all Inpatient admission requests across the Wigan Borough.
  • Effective liaison with inpatient units across the Trust to facilitate early discharges or to undertake reviews of those users placed out of area.
  • Duty Practitioner /2nd Duty, as part of a rotational roster.
  • Working in a professional and collaborative manner with the multi-disciplinary team.

Access to transport for the post is essential due to the majority of the interventions taking place in the community setting.
