Job Details

The Suffolk Secure Services Forensic Community Mental Health Team provide recovery-focused community care. This is for offenders who have been, or are being, discharged from a secure unit.

Hi, I'm Percy, the Secure Service Forensic Community Team Matron. Our community services have been awarded some collaborative funds for the next 12 months to focus on service user progression and achieving the best for the service user by reducing the risks of re admission to secure sittings, reducing length of stay in secure settings and supporting with more community resource to aid recovery and step down from secure settings.

We are looking for a highly skilled, motivated, and passionate Band 6 Forensic Community Nurse to join our Suffolk Team for a period of 12 months (please note there is a similar advert for Norfolk Team). This is a team with a wealth of experienced colleagues who are focused on recovery and making a difference thereby has a positive impact on the quality of life for our service users.

Main duties of the job

We are openly welcoming people from a diverse clinical background who poses a vast experience in different areas of mental health. Your wealth of skills and expertise in metal health will be valued and career progression will be supported within our line management and appraisal structures.

Within this role you will be involved in working alongside the MDT managing cases, hold an individual caseload, working within the confines of a robust co-produced care plan that meets the individual service users' needs whilst managing any risk factors for the individual or others. The multidisciplinary team is recovery focused and works with a mixed caseload of males and female service users who have complex mental health needs. Working with carers, relatives and other third-party agencies will be a very crucial part of your role.

In addition, this role will focus on linking with services users placed out of area and proactively supporting plans to return them to the local area, in reaching inpatients services, attending complex case meetings, supporting reintegration back into the community, linking with third party services such as housing, drug and alcohol, delegating specific tasks to the allocated band 4 or 5, attending all the necessary reviews and professional meetings as scheduled.

**This post attracts a recruitment premium of £3,000 (pro-rata for part time roles). This is a one off payment with half paid on appointment and half paid after the completion of a satisfactory probation period (6 months). The premium is taxable. Please note that should you leave employment, or choose to move to an alternative post within the Trust that does not attract this recruitment premium, you will be required to reimburse all or some it, as follows: 100% of the value of the premium in the first 12 months; 50% of the value of the premium in the period 12 months to 2 years following appointment. Please note this recruitment premium is for external applicants only. Not applicable to newly qualified nurses**

By joining our team, you will be well supported to develop further your knowledge and skills to deliver evidence-based interventions for the group of service users. Personal development will be supported through meaningful Appraisal and via a bespoke training package (a comprehensive in house & external training programmes). On a monthly basis you will be offered reflective practice sessions, mandatory clinical supervision, and Line management supervision. You will also have an opportunity to actively take part in Quality improvement projects within the service
