South Later Life CMHT are looking to recruit 2 Band 6 Community Mental Health Nurses to join a busy Multi Disciplinary Team. The post offers the opportunity to work within the CMHT and the Memory Assessment/Treatment Service. Full training will be provided around the Memory Assessment pathway.

There is the opportunity for professional development within this role.

Regular Management and Clinical Supervision is provided.

Main duties of the job

An opportunity has arisen for 2 qualified and experienced individuals to join a busy and vibrant Community Mental Health Team for Later Life based in South Manchester. The successful candidates will be required to fulfill the role of a CPN as part of a multi-disciplinary team; the care co-coordinator role is an essential component of the role. The team operates from Etrop court, Wythenshawe Manchester, M22 5RG.

You will be responsible for the management of a case load of clients with complex functional and organic mental health issues and associated social care issues, for which risk assessment and risk management is an essential requirement. In addition you will also assess people with memory problems and offer support following diagnosis. You will also participate in the Duty rota
