The Andover Adult CMHT is looking for an enthusiastic, dedicated and talented professional to join the team. As this role is community based, applicants will need to hold a full UK driving licence (or equivalent) and have access to a suitable vehicle for work.
Main duties of the job
To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.
To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.
To undertake ongoing assessment, treatment and evaluation for service users known to the service
To support the delivery of therapeutic interventions either individually or within a group setting.
Support the transformation of the team to deliver local evidence based care.
