A fantastic opportunity has arisen to be part of our well-established Community Nursing teams in Wokingham or Bracknell, as a Community Matron / Enhanced Community Nurse Practitioner.
The post holder will work full time within an integrated community nursing team to proactively manage a complex caseload of high-risk patients, applying advanced clinical skills, clinical reasoning and practising autonomously.
The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive self-management of long-term conditions with the aim of avoiding crisis resulting in unplanned hospital admission.
The post holder will work be fully integrated into the locality community nursing team, working with other stake holders to manage complex patients, as well as the wider multi-disciplinary team in a variety of settings to improve health outcomes and quality of life for patients with multiple chronic diseases.
Main duties of the job
The role requires the applicant to take a lead role in managing patients with complex needs to avoid hospital admission and access services as required. The post holder will be a senior nurse supporting community nursing using established independent prescribing and advanced assessment skills, upskilling junior members of staff and supporting scheduled caseloads
