We are looking for a Band 6 (experienced band 5 with view to progressing to band 6 will also be considered) Community Physiotherapist to join the Community Rehabilitation Service based at Minehead Community Hospital in West Somerset.

The post will cover the Rehabilitation ward at Minehead Hospital plus a varied community based caseload including Frailty, Amputee Rehab, Neurology, MSK community based conditions, Orthopaedics, Falls, Respiratory and more....you really can never get bored with community work!

The team includes Physiotherapists, OT's, Rehab Assistants and Pharmacy Technicians. We also have direct relationships with PCN OT's, Enhanced Clinical Practitioners, Advanced Clinical Practitioners in Frailty and Rehabilitation, Specialist Nurses in Neurology, GP's and Acute Consultants, and many more.

The Community Rehabilitation Service can offer and support opportunities for you to develop and progress along your chosen career path, whether that be clinically based, through the ECP/ACP pathways towards a band 8A ACP role, or a move into leadership and more senior management positions.

Car driver and own vehicle is essential to this post.

Main duties of the job

  • To provide high quality patient-centered care and specialist rehabilitation to adults referred to the Community Rehabilitation Service.
  • Participate in the development of the Rehabilitation Service in the community.
  • To provide specialist advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.
  • Provide support, guidance and training to Band 5 Therapists, Senior Rehabilitation Assistants, Rehabilitation Assistants, Apprentices and Students.
  • Responsibility for a caseload of complex patients within the community setting.
  • Act as Clinical Supervisor and undertake Managerial Supervision including Appraisal for Band 5 Physiotherapists and Rehabilitation Assistants.
  • Act as Team Lead in the Rehabilitation Managers absence, including supporting staff and attending any meetings delegated by the manager

Planning and Organisation
  • To use advanced clinical reasoning to predict achievable outcomes of treatment and to recommend the best course of intervention, developing comprehensive treatment plans, in conjunction with other professions and agencies.
  • To be responsible on a daily basis for the allocation of clinical work to the Rehabilitation Assistants
  • To manage clinical risk within own patient caseload and take action to effectively manage identified risks.
  • To be responsible for organising, prioritising and planning your own caseload and that of the staff under your supervision within the Community Rehabilitation service

  • To be able operate appropriate Information and Digital Technology systems to contribute to record keeping and data analysis, under guidance from Trust Policy.

Responsibility for Patient / Client Care, Treatment & Therapy
  • To use specialised skills to provide assessment and treatment of patients who may have diverse presentations and complex physical and psychological conditions
  • To provide a high standard Physiotherapy service to all patients referred to the service.
  • To develop and deliver an individualised treatment programme
  • To agree patient centred goals in partnership with patients, carers, and other members of the healthcare team.
  • To provide specialist advice, teaching and training regarding rehabilitation management of patients on caseload.
  • To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care, (including care delegated to students and assistant staff).
  • To ensure a high standard of clinical care for the patients under your management, and support more junior staff to do likewise.
  • To undertake and carry out a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies
  • Use advanced/specialised clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition or their level of function.
  • To regularly work alone in unpredictable environments e.g. patients own home, where there may be an element of risk e.g. dealing with verbal aggression. This may include working in unclean surroundings and coming into contact with dirt, fleas, lice, passive smoking etc.
  • To represent the Community Rehabilitation Service and/or individual patients at multi-disciplinary/community team meetings, providing specialist information/ advice, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate Rehabilitation into the treatment programme.
  • To evaluate patient progress, reassess and alter treatment programmes if required
  • To formulate and deliver an individual Physiotherapy treatment programme based on extensive experience and a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of treatment skills.
  • To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of their condition. Communication skills of persuasion, motivation, explanation to encourage patients to undertake their treatment programme.
  • To use communication tools (verbal and non-verbal) to gain valid informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, altered perception, expressive and receptive dysphasia, pain, fear. To have the capacity to work within a legal framework with patients who lack capacity to consent to treatment e.g. those with cognitive or speech difficulties.
  • To be aware of and appropriately manage sensitive and/or complicated information. To deal sensitively with patients who may have challenging behaviour, have high levels of anxiety and aggression caused by pain, dementia, limited mobility, terminal illness, degenerative disease etc.
  • To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time. This may primarily be community based but will require cover for other patient groups during times of staff absence for holidays, sickness, etc. Within the working day, there can be frequent interruptions to the work pattern.
  • To carry out assessment and treatments of a range of medical conditions, which may involve manual physiotherapy techniques with, moderate physical effort on a daily basis.


Policy, Service, Research & Development Responsibility
  • To undertake therapeutic moving and handling techniques with patients e.g. normalising tone, functional mobility, requiring moderate/intense physical effort on a frequent basis for short periods. This can include long term immobility and morbidly obese patients. This will take place in awkward and confined spaces such as hospital wards, home/work environments and cars and will include moving and handling equipment, wheelchairs and specialist therapeutic equipment as part of treatment programmes.
  • To be responsible for the safe and competent use of all physiotherapy equipment and patient appliances. This will include assessment and prescription of equipment from the community equipment service, to take account of patient need, financial resources and instruction in safe use
  • To comply with the Trust Manual Handling Policy and local therapeutic handling guidance at all times.
  • To provide written and verbal reports to members of the multi-disciplinary team during treatment and at discharge from the service. To be responsible for the safe and competent use of all physiotherapy equipment and patient appliances. This will include assessment and prescription of equipment from the community equipment service, to take account of patient need, financial resources and instruction in safe use

  • BSc Hons Degree in Physiotherapy
  • HCPC Registration
  • Current permit to work in the UK
  • At least 3 years post graduate experience in a relevant field
  • Car driver with access to fully insured car and current UK driving license
Desirable
  • Previous leadership experience including appraisal and supervision
  • Previous experience in community rehabilitation
