The KCHFT Lymphoedema service is a Kent wide specialist service and has 4 main clinics covering those CCG localities with additional satellite clinics to provide care closer to the patients' home. Although you will be based within one main lymphoedema clinic there will be an expectation for you to travel to and work from other clinic sitesaccording to the needs of the service.

You will have a skilled support network which includes regular interaction within the wider team and team meetings allowing for shared growth, learning and development.

Main duties of the job

This is a specialist role to facilitate excellent standards of lymphoedema care to a defined patient population. Youmust be able to demonstrate a range of high quality, clinically robust,evidence based lymphoedema skills to assess, treat and plan appropriate care for patients with lymphoedema. Youwill have the overriding responsibility for a case load and must be able towork without direct supervision although you will have the guidance of the Clinical Lead and Operational lead if required.

The overall aim of the lymphoedema service is to empower patients to self-manage their lymphoedema on discharge from the service you will be directly responsible for ensuring that clients are discharged in a timely manner towards self-care and for identifying and implementing treatments that will improve outcomes and quality oflife for patients with lymphoedema

Additional Working Pattern: 4 days around clinic room availability on all 4 kent sites m-f 9-5 and home visits

Person Specification

QUALIFICATIONS

Essential

  • Registered health care professional (NMC or HCPC registration ) / minimum of 5 years of experience in health improvement environment (delete as appropriate) Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Specialist qualifications relevant to area of practice ( examples -:non-medical prescribing mentorship , coaching , clinical expertise)-


Desirable

  • PRINCE 2 or equiv project management training PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study Working towards Master level study


EXPERIENCE

Essential

  • Minimum of 5 years' experience in area of practice Practical experience in managing health related projects/complex caseloads Supporting others (mentor / educator / supervisor ) experience


Desirable

  • Experience of leading and managing teams Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ RIO)


SPECIAL KNOWLEDGE AND EXPERTISE

Essential

  • Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services Evidence of continued relevant professional development and implementation in practice within the last 12 months

Essential

  • Understanding of training needs in relation to post Ability to manage time effectively, priorities and work under pressure to meet deadlines Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation Ability to communicate complex, sensitive information to individuals and groups Strong influencing and negotiating skills
