To be a highly specialised nurse working with a specified group of adolescents/adults with cystic fibrosis (CF) and taking responsibility for their ongoing clinical management at home and in hospital.
Acting as a carer and advocate for those families and giving expert support and advice based on best practice with close communication with the adult cystic fibrosis multidisciplinary team.
To act as an expert in cystic fibrosis both locally nationally and internationally.

Main duties of the job
To provide both a hospital and homecare service for adolescents/adults with CF. To work closely with colleagues in the CF Homecare Team providing direct care, education and support for patients and act as a specialist resource when liaising with and facilitating the best use of all available services.
To assess, plan, implement and evaluate programmes of care (both in hospital and at home) for adolescents/adults with CF. For example, intravenous therapy, end of life care, psychosocial and respiratory support.
To act as an expert resource for adolescents/adults and their families, giving advice, education and support that is appropriately planned to meet each individual's needs empowering them to manage their treatment regimes, to recognise any deterioration or acute episodes requiring immediate specialist attention and how to access the necessary support.
To participate as appropriate in a seamless transition of care for adolescent patients with CF and their families from the paediatric CF multidisciplinary team to the adult CF multidisciplinary team.
Run the annual review service (including seeing patients, coordinating service, entering data, preparing reports) with assistance from the annual review administrator and other MDT members.
To provide support and cover for colleagues as necessary.

We are a dynamic multidisciplinary team delivering holistic care and are actively engaged in research, clinically focused quality improvement, service development and audit. To work effectively within the Trust's adult multidisciplinary CF team as required (including attending ward rounds, meetings and out-patient clinics as appropriate) seeing in-patients as necessary and ensuring an individualised, evidence-based optimum service for each patient and their family.
To always communicate closely with members of the CF MDT.
To act as advocate for the adolescent/adult and family enabling them to express their needs effectively when dealing with the multidisciplinary team.
To liaise regularly with the CF Advocates at the Royal Brompton Hospital.
PEOPLE MANAGEMENT
To develop and maintain clinical skills and knowledge of the speciality of CF that are evidence-based aiming to achieve a 'best practice' principle.
To mentor post-registration, pre-registration students and visitors to the CF Team as requested.
To participate in the in-house education programme and to undertake informal teaching of ward nursing staff as required.
To act as a role model and clinical expert for staff providing education as appropriate.
To work in partnership with colleagues, having accountability for decisions made (in accordance with the NMC Code of Professional Conduct).
To develop professional practice through reflection, participation on post registration courses and attendance at meetings and conferences.
To identify and be responsible for own professional and managerial development in liaison with the Lead Clinical Nurse Specialist, Consultant Nurse and CF Centre Director.
SERVICE DEVELOPMENT
To ensure the constant provision of effective, high quality nursing care to all patients with CF.
To support and work with the CF multidisciplinary team in the introduction of clinical changes.
To be actively involved in the development of CF-related protocols and procedures in conjunction with other members of the CF multidisciplinary team.
To keep up to date with new advances in treatment and new developments in the world of CF research.
POLICY DEVELOPMENT
Be aware of and act in accordance with the Patients Charter for the care of patients with CF.
To be aware of and act in accordance with the CF Trusts National Guidelines on CF Care.
To participate promptly and effectively with the investigation of complaints and incidents in line with Trust policy and procedure.
To maintain high standards of nursing care within own practice and monitor the standard care for the adolescent/adult requiring admission in collaboration with senior ward nursing staff.
INFORMATION MANAGEMENT
To ensure accuracy of documentation and to maintain an efficient and accurate record system with relevant information in agreement with the CF team.
To be responsible for data entry and maintaining patient databases e.g.: online annual review forms, patient contact logs and homecare databases.
To access, record and utilise patient information through our electronic patient record system.
Ensure self and team undertake training for electronic information systems in place & under development.
To have an active email account and use this as one of the methods of disseminating information to the wider team.
RESEARCH & AUDIT
To maintain an active awareness of clinical and professional developments within CF and to disseminate that knowledge to all colleagues.
To identify and suggest relevant areas for research within adolescent/adult CF nursing which will benefit patient care taking the lead where appropriate.
